Spc Nature: PUNCH BIOPSY

Spc Site: ANAL VERGE

History: Per rectal bleeding, abdominal distention with diarrhea, history of weight loss 10kg, a fragile irregular
mass 5 cm from anal verge.

Gross: Specimen container is labeled with the patient's name and medical record number.
Received in formalin is a single nodular fragment measuring 11 x 9 x 6 mm. The entire specimen is
submitted in a single block.

Micro: Sections reveal a polypoidal tissue fragment showing tubular structures lined by high grade dysplastic
epithelium. No definite invasion is noted.

Diagnosis: ANAL VERGE, PUNCH BIOPSY:

Tubular adenoma with high grade dysplasia.
No invasive tumor seen.
