An evaluation report is a paper that examines whether a product, service, or process is working,

according to a set of standards. It includes a title page, executive summary, table of contents and other
sections that preface the report, introduction and background, methodology and summary, conclusions
and recommendations.

On the title page, the title is sufficiently clear and concise to facilitate indexing. Authors names and
affiliations are also identified. The date of preparation must be included. Title identifies what was
evaluated, including target population, if applicable. The name of client of funder(s) is identified. Text
and material on title page are clearly abs and properly arranged.

Secondly, the Executive summary contains a description of the program or project. Evaluation questions
and purpose of evaluation are also included in the executive summary. Also, there's a brief description
of methods and analytical strategy, if appropriate. The executive summary contains summary of main
findings, implications of findings. It also contains recommendations, if appropriate.

As for the Table of Contents and Other Sections that preface the Report, the Table of Contents contains
at least all first and second level headers in the Reports. Title and page numbers are accurate. List of
tables, figures and appendices are included, if appropriate. List of acronyms or abbreviation is included,
if appropriate. Also there's acknowledgments section, references, sponsors, data collectors, informants,
contributors to the report, research assistants, reviewers of the report e.t.c.

The Introduction and background is also part of an evaluation report. It contains the purpose of
evaluation and evaluation questions, if not covered in the methodology section. Also, it contains
description of the program or project or phenomenon being evaluated including goals and historical
content, if appropriate. There's identification of target population for the program and relevant
audiences and stakeholders for the evaluation. Under introduction and Background we also find review
of related research and finally overview and description of report structure.

Another major content of an evaluation report is the Methodology. It contains the purpose of evaluation
and evaluation contents if not covered in the introduction. Evaluation approach and model being used,
as well as rationale for the approach or model are also included in the Methodology. We also find Design
of Evaluation, including sample sizes and timing of the data collection. There are also methods of data
collection including description of data collection instruments. Sources of information and data are also
presented under methodology. Lastly, we find limitations of the evaluation e.g limitations related to
methods, data sources, potential sources of bias etc.

Summary, Conclusion and Recommendations also being a major content of evaluation report, it contains
summeries of findings in each chapter or altogether in a summary chapter. Discussion and interpretation
of findings are included. Summary and conclusions fairly reflect the findings. Judgements about the
program that cover merit and worth are included. If appropriate, recommendations are included and are
based on findings in the report.
