AUTHORIZATION LETTER

KNOW ALL MEN BY THESE PRESENTS:

I, EVA MAITOM, of legal age, Filipino and a resident of Malvar St., Davao
City, do hereby appoint , of legal age, Filipino, married and a resident of Bajada,
Davao City as my true and legal representative to act for and in my name and
stead to perform the following acts:

1. To authorize, follow-up with the application of my father and mother Birth,

Marriage and Death Certificate;
2. To execute, sign, process, secure process, release and submit whatever
documents and pay all the fees required or necessary to effect the
performance of the above acts in communication with the authority given;
3. Hereby granting unto my representative fall power and authority to execute
and perform every act necessary to render effective this authorization as
through ourselves personally present has no performed it and hereby
appearing all that he may do by virtuesMN hereof with full right of
substitution of his person and revocation of this instrument.
