Harmonization guidelines, and applicable regulations and laws;

 I agree to obtain, in the manner described in the approved research proposal, written informed consent
or informed assent to participants in this study;

 I am aware of the requirements for the correct reporting of serious adverse events, and I commit to
document and to report such events as required;

 I agree to supply, upon request, The Rizal Memorial Colleges, Inc. with evidence the name and address
of each participant/respondent of the study;

 I agree with the use of results of the study for the purposes of national and international registration,
publication, and information for academic and education professionals;

 I agree to keep all source documents and case report forms as specified in the research proposal;
