INTRODUCING CIMPLE HEALTHCARE MANAGEMENT PLATFORM

Further to your previous conversations with our representatives on the above

subject, we hereby formally apply to be appointed as your Members’ Healthcare
Insurance Management Partner, using our healthcare management platform
(cimplehealthcare) to enroll your members for various healthcare insurance plans
offered by our Healthcare Insurance Partners (HMOs), as may be agreed with you.

The key objectives of our platform are to:

 increase healthcare outcomes while reducing healthcare cost, and
 improve healthcare operational efficiency and data transparency.

The following are some of the key operational activities that will be handled through
the platform:
 registration of interested members for approved Health Insurance Plans – both
self-registration and assisted (3rd party) registration.
 verification and approval of registered members (beneficiaries) by authorized
representatives of your organisations – based on your predetermined rules.
 payment of agreed premiums for the verified/approved beneficiaries – either
100% borne by your organization or co-paid (shared).
 transfer of beneficiary details and related premiums to the concerned Healthcare
Insurance Providers for effective enrolment in the approved/agreed schemes.
 services and activities statistics, including management dashboards.
 where applicable, tracking and reporting of the:
 rendition of healthcare services by approved hospitals and clinics,
 dispensing of drugs by approved pharmacies, and
 referral of patients by approved hospitals and clinics.

As you may have been informed, we are one of Hygeia HMO Ltd (Hygeia)’s
approved Enrolment Partners and we have successfully integrated our healthcare
management platform with Hygeia for purposes of providing the required services to
our partners. We currently provide the services to Itsekiri Trust Fund Healthcare
Scheme, with Hygeia as the approved HMO.

Furthermore, we are in discussion with other HMOs for similar partnerships.

Meanwhile, it is worth noting that you can also use our platform for general
membership nominal roll management, if required at any point in time.
