WFH Enablement One-Time Reimbursement

This document is being executed to attest that the Assets1 described were purchased in accordance with
the WFH Enablement One-Time Reimbursement. I, _______________________________ [name of
employee] with _______________________ [eid], hereby voluntarily undertake and attest that:

1. I fully understand and agree that the Assets are intended to be used primarily for my work in the
Accenture, Inc (“Company”). and not for personal benefit.
2. I fully understand and agree that my entitlement to the Assets under the Program is conditioned
on my continued employment with the Company for one year upon hire date.
3. Assets are valuable assets, so it is my responsibility to safekeep and secure it at all times.
4. I likewise warrant my adherence and compliance to all Company policies on the use of the
Assets regardless of place of work.
5. I hereby expressly authorize the Company to deduct from my salary, final pay, or any amounts
due to me from the Company the amount needed to cover the cost in case of breach of any of
the terms and conditions of this Attestation Form and/or the WFH Enablement One-Time
Reimbursement.
6. I hold the Company free of any legal obligations, including but not limited to, timely release of
my final pay as result of delays in settling my accountabilities.

I attest that I have read, understood and confirm all the terms set out in this Attestation Form, including
Company’s authority to deduct from my salary, final pay or any amounts due me from the Company. I am
executing this document voluntarily, out of my own volition and free will, without any force, intimidation,
duress or improper pressure to vitiate my consent.
