NSC 395B Worksheet #3: SMART Goal Setting 20 pts

OBJECTIVES
● To help clients develop meaningful long-term and short-term SMART goals (HC Manual, Chapter
13).
● To identify potential obstacles and how to overcome them.

DESCRIPTION
This worksheet will provide you an opportunity to practice creating Specific, Measurable, Attainable,
Relevant, and Time-bound (SMART) goals for clients.

Long-term SMART goals are those specific, measurable, attainable, relevant, and time-bound goals that
you hope to have achieved in the next 6 months to one year and beyond. These are the goals that you
will make slow and steady progress toward achieving each time you achieve your short-term SMART
goals. The short-term goals are measured in days, weeks, and months. Read the article “A Guide for
SMART Goal Setting” Complete the following questions in the table below.

Why are goals essential to success for a client? 2 pts

They give the client a purpose and reason to become successful at their endeavor

Rephrase the following goal into a SMART Goal: 2 pts

“I want to eat healthier”

I will eat 3 servings of green vegetables per day for the next 6 weeks in order to have a
better quality of life

Rephrase the following goal into a SMART Goal: 2 pts

“I want to start exercising”

I will start working out 5 days per week for the next 6 weeks in order to build 5 lbs of muscle

Role Play: Using the Client Goal Questionnaire from the HC Manual, Chapter 13, page 363,
practice asking a friend, family member or spouse a few of these questions on the list. Were
you able to draw out any unexpected details from them and how may these answers help you
move forward with goal setting (in the next question)? 4 pts

Role Play: Ask the same friend, family member or spouse about their health and fitness
related goals. From that goal, reframe and create at least 2 short term and 2 long term
SMART Goals for them (4 SMART Goals total): 8 pts
I will read 1 book on real estate per month for the next 12 months in order to get my real
estate license

I will go to sleep by 7pm every night for the next 2 weeks in order to wake up for the gym at
4am every morning

I will eat 5 servings of green vegetables per day for the next 5 weeks in order to have a
better quality of life

I will start working out 3 days per week for the next 3 weeks in order to build 3 lbs of muscle

Once you have created these SMART Goals for your “client”, talk about any internal or
external obstacles they may face along the way. Give at least 2 ways they can face and
overcome either an internal or external obstacle that they identify in relation to their goal? 2
pts

Internal obstacles would include lack of motivation, drive, and consistency. One way to fix
this is through creating a routine. Another way to stay on track is by writing down their goals
every day

External obstacles can be a busy schedule, but proper planning prevents poor performance so
meal prepping is very helpful as well as creating a daily schedule.
