Remarks:-

1) To submit booking form at least 3 working days in advance for Hong Kong/Guangdong/Shanghai province, and 5 working days in
advance for other areas (all Saturdays, Sunday & public holiday not included).

2) Make sure that 80% of the goods have been packed and 100% of the goods have been finished upon inspector(s) arrival, otherwise
the inspection may be worked overtime or cancelled and supplier/factory has to be required to pay the abortive inspection cost,
which is the inspection fee quoted.

3) For inspection on Saturday, 1.5 times of normal man-day rate. For inspection on Sunday, 2 times of normal man-day rate.

4) Any booking within the period of Lunar New Year Holiday, China May 1st Labor Day Holidays and China Oct. 1 National Day
Holidays should be at least 2 weeks in advance. Eurofins reserved the right to accept the booking, depends on availability of staff.
The man-day fee in such period will be 3 times of that of normal man-day rate.

5) Overtime charge (= man-day rate / 8 hours) will be charged per hour after normal working hours and will be counted from 6:00pm
onwards. Less than 1 hour will be counted as an hour. This is applicable to weekday, Saturday, Sunday and public holiday.

6) Handling fee of USD350 will be charged to the supplier/factory for any postponement or cancellation request made less than ONE
working day of the scheduled inspection date. In case, for inspection location which requires our inspector to travel by air, and air
ticket has been made, the client should be responsible for the air ticket charge. The working day of Eurofins is 9:00am to 6:00pm
and from Monday to Friday (Hong Kong public holidays are not included).

7) Re-send this form for all amendments made and also put indications on the amendment area on the top of this sheet.

8) Follow up your booking by phone call to (+86 21 61819181) (inspections) to ensure that we have received your booking.

9) The liability of Eurofins in respect of any claim for loss, damage, or expenses of any nature shall be arose by written notice within 3
months from date of inspection completed howsoever in no circumstances exceed a total aggregate sum equal to 10 times the
amount of the fee paid in respect of the specific services which give rises to such claim or USD 10,000 whichever is the lesser and
Eurofins shall have no liability for any indirect or consequential loss (including loss of profits).

We declared that the above information given by us is true and correct. We agree that the inspection be carried out in accordance with an
agreed inspection plan and Eurofins has the full discretion to carry out the inspection. We agree that the inspection certificate or report is
issued on the understanding that it cannot form the basis of, or the instrument for, any legal liability against Eurofins.
