SURVEY QUESTIONNAIRE

This questionnaire is intended to gather data regarding the impact of Joint


Development Voucher Program of Technical-Vocational-Livelihood Track (JDVP-TVL)
to the attitude and skills of Grade 12 students in Libertad National High School. This
survey will collect data about the profile of the students such as their names, ages,
sexes, strands, and the date of implementation of JDVP-TVL. This will also collect the
data about the availability of resources in the JDVP-TVL and lastly, it will collect data
about the attitude of the students towards the said voucher program.

Part I. Profile
Republic Act No. 10173 or the Data Privacy Act of 2012 (DPA) “to protect the
fundamental human right to privacy of communication while ensuring free
flow of information to promote innovation and growth [and] the [State’s] inherent
obligation to ensure that personal information in information and
communications systems in government and in the private sector are secured
and protected”
