A. AUTHORITY:

01. Verbal Order from Mr. Herbert G. Ballego APMO Asst. Manager SBU3-PCFI
Iloilo, on 05 January 2021 for the undersigned to conduct investigation
regarding the above cited incident.

B. MATTERS TO BE INVESTIGATED:

02: To determine the facts and circumstances surrounding the alleged missing
items at the unit of HO Nadine Ibanez of PRJ 0307.

C. FACTS OF THE CASE:

03.That on or about 1320H 05 January 2021, arrival of Mr. Niel Ibanez SPA of
HO Nadine Ibanez of PRJ Block 03 Lot 07 went to UngkaOffice and complaint
alleging missing two (2) rolls of electrical wires.

D. DISCUSSION:

04. Upon knowing, the undersigned immediately called thru radio SIC Hembrato
conduct initial investigation on said complaint of Mr. Ibanez.

05.Immediately, the undersigned call the attention of roving guard on duty Security
Guard Aladin T. Taton to proceed to said area.

06.Therefore, to verify the said complaint, 1332H 05 January 2021,the undersigned

and SIC Hembra proceed to investigate in the area.

07. Initial investigation conducted disclose that based on the statement of Mr.
Marco Gayon worker/key holder that on or about 1200H 31 December 2020
Thursday, they left and close the unit together with his four (4) companion
namely: Mr. MargaritoGayagoy, Mr. Christopher Mandras, Wilfredo Taay and
Certain Jonas. They left the unit key to their boss and contractor Mr. Alvin
Villapando of Providence before going home to their barracks at BrgyMaliao,
Pavia.

08. Further, On or about 1300H 04 January 2021 Monday, they return to the unit
for work only with Mr. Gayagoy& Mr. Taay upon opening he found out that the
back sliding door was slightly open and also found out that the following items
were missing,15 meters of THHN # 6 and 30 meters of THHN # 12.

E. CONCLUSION:

09.After a thorough investigation to SPA Ibanez and their workers, therefore we

conclude that the above initial investigation that the unknown person forcibly

opened the sliding door at the 2 nd floor and used as his entry going down
stairs.(see attach picture at the back for references.)

F. RECOMMENDATION:

10. It is recommended to the homeowners and workers to secure their valuable

materials so that it will not happened again.

11. Also recommended that if possible the home owner or contractor will let one of
his workers to stay in or bring their power tool when they go home.
