C-H= Critical Hazard Deficiency A deficiency in a component or element of a bridge that poses an extreme hazard or unsafe condition to the public, but does not impair the structural integrity of the bridge. Examples
include but are not limited to: Loose concrete hanging down over traffic or pedestrians, A hole in a sidewalk that may cause injuries to pedestrians, Missing section of bridge railing,
etc.

REMARKS
BRIDGE ORIENTATION
This bridge is oriented from west to east over the Fort Point Channel which is tidal.
GENERAL REMARKS
The Evelyn F. Moakley Memorial Bridge, B-16-223 (AWQ), is a five span structure which carries Seaport
Boulevard over the Fort Point Channel (see photos 1 through 10).
The superstructure consists of two prestressed concrete I-beam west approach spans (spans 1 and 2), three
prestressed and post-tensioned concrete spread box beam main spans (spans 3 through 5), and a 3 span
continuous two directional deck slab across the top of a nine chamber (3 x 3) east abutment vault. The
prestressed concrete I-beams are numbered from 1 through 14 in span 1 and 15 through 28 in span 2 from
north to south. The 3-span continuous prestressed and post-tensioned concrete box beams (spans 3 through
5) are numbered from 29 through 38 from north to south, with the beams named the same through all three
spans (see sketch 1).
The spans and piers are numbered from west to east (see sketches 1 and 2).
The substructure consists of a stub abutment (west abutment), two pile bents with reinforced concrete caps
and piles composed of reinforced concrete filled steel pipe with fiberglass jackets (pile bents 1 and 2), two
reinforced concrete piers with masonry pierwalls (piers 3 and 4), and a nine chamber reinforced concrete
vault with chambers oriented three wide by three long (east abutment). These chambers are arranged in a 3
x 3 pattern, and are named north, center, and south transversely, and west, center, and east longitudinally to
the roadway (see sketch 3). This nomenclature system follows the convention set in the design plans.
The east abutment vault is accessible through square manholes in the north and south sidewalks, with fixed
ladders mounted to the chamber walls. The manhole covers weigh approximately 300 pounds each and are
very difficult to open. Three large crow bars were required to pry open the north manhole cover and slide it
along the support frame and onto the sidewalk, taking care to keep the cover parallel to the support frame.
The manhole was gated off and a manhole attendant was used to guide pedestrian traffic while the cover was
open.
There are numerous openings for air circulation along the west wall of the vault and two unlocked hatch exits,
therefore, the vault is considered a non-permit confined space. There was 12" of standing water in the
western three chambers of the vault. Waterproof boots or knee high waders are required to access these
chambers.

REMARKS
Span 5
- North bike lane: three areas of hardened concrete debris bonded to the concrete wearing surface;
9" diameter by 1" high near pier 4, 1'-4" wide by 7'-0" long by 3/4" high just west of midspan and
3'-4" wide by 8'-0" long by 1 1/4" high east of midspan (see photo 11).
- Left westbound lane at the west quarter point: a 2'-0" wide by 2'-0" long by 3/4" high area of
hardened concrete debris bonded to the concrete wearing surface.
The wearing surface has moderate wheel line rutting throughout. Some of the traffic markings are faded and
have chipped and missing paint (see photo 9).
There is a 6" wide by 9" high by 2'-0" long edge spall with exposed reinforcing on the roadway face and a 6"
wide by 7" high by 3'-0" long edge spall on the sidewalk face (see photo 16) of the north curb on the east
approach.
The curb reveals were measured to the top of the concrete section of the BR-2 traffic barrier.
South Sidewalk
- Span 3, near midspan: A 1'-4" wide x 1'-7" long delamination.
- Span 3, at pier 4: 3'-3" long x 15" wide hollow area with 17" long x 8" wide x 1-1/2" deep spall (see
photo 17).
- Span 4, at pier 4: A 3'-0" wide x 2'-4" long x 1-1/2" deep hollow area/spall (see photo 18).
- Span 4, at the east quarter point of the span: A 5" wide by 8" long by 1/2" deep spall with rust stains
at a location with a sign post cut flush with the bottom of the spall.

- East abutment vault, at the east end: A 1'-0" wide by 1'-2" long area adjacent to the curb is breaking
up and heaving 3/4".
North Sidewalk
- Span 3, near midspan: a 6" wide by 6" long by 1" deep spall with full length of span 3 longitudinal
hairline crack (see photo 19).
- Span 4: longitudinal hairline crack x full length of span (see photo 20).
- East abutment vault
o At the western quarter point: a 1/16" wide transverse crack across the full width of the
sidewalk.
o At the vault access manhole: small spalls around the perimeter of the manhole frame and
hairline cracks radiating from the corners of the manhole frame (see photo 21).
- Span 1, north traffic rail: the end of the rail was repaired in 2011 by cutting it off and bending it
downward without an attachment. Two bolts protrude from the end of the adjacent curb at the
location of the original rail end.
- East abutment vault at the finger joint, south traffic rail: the traffic rail has a misaligned sliding
expansion insert with adjoining tube rails disconnected from each other. Additionally, the tube rails
are misaligned laterally 3/4" and the insert does not line up (see photo 23).
Pedestrian Railings
The pedestrian railings typically have areas of chipped and heavy paint loss throughout the rails, posts, and
rods (see photo 24), with areas of light rust forming on the rods. Isolated additional deficiencies are as
follows:
- Span 1, north pedestrian railing, western two pedestrian rail posts: there is 1 of 4 anchor bolt nuts
missing.
- Span 2, north pedestrian railing, near the east end of the span: the cap stone for the pedestal/post
is spalled 7" wide by 7" long by 3 1/2" deep.
There are illuminated glass and steel spires on the pedestal/posts over piers 3 and 4. There are steel spires
(no glass, not illuminated) at the four corners of the structure. There is minor peeling paint along the spires.

REMARKS
Below the deck the drainage system has isolated deficiencies as follows:
Span 5
At the eastern quarter point of the span between beams 36 and 37:
- The lower PVC drainpipe elbow has broken off and is missing; however, the vertical section of
drainpipe still discharges beyond the beam bottom flanges.
The west lighting standard at the east abutment vault on the north curb has been removed. The base of this
light post is roughly cut with a small heavily rusted stub remaining (see photo 28). In addition, there is a
fracture in the adjacent utility box cover for this abandoned lighting standard.
- Span 3, north curb, second lighting standard west of pier 3: a 9" wide x 5" high x 2'-0" long edge
spall with light standard removed (see photo 29).
- Span 5, north curb, second lighting standard east of pier 4: a 6" wide x 9" high x 2'-6" long edge
spall with exposed reinforcing (see photo 30).
- Span 5, north curb, first lighting standard west of the east abutment finger joint: a 7" wide x full
height x 2'-0" long edge spall with exposed reinforcing (see photo 31).
- East approach, north curb at abutment vault slab: a 5'-0" long x 2'-0" wide x 8" deep spall with
exposed reinforcing and baseplate (see photo 32).
The lower portions of the lighting standards have section loss as follows (DEF=S/A):

Interior of the East Abutment Vault
The utility support beams have clip angles connecting the webs of the utility support beams to the vault walls
with missing bolts; however, these utility support beams are fully seated on concrete pedestals or steel seat
angles (see photo 39). The utility junction boxes in the western chambers are heavily deteriorated, some with
open covers, exposing wires within (see photo 40).
In the center-east chamber, there is a pair of nitrogen utility lines, each with a dial gauge. One gauge reads 8
PSI and the other is over the gauge limit of 160 PSI. The tags read "this line is charged with 20 PSI nitrogen,
do not open" and "this line is under 20 PSI pressure, do not open, nitrogen", respectively (see photo 41).
There are shut off valves in the off position of each gauge. This condition was reported to MassDOT during
the 2018 inspection.
See sketch 3 for specific utility deficiencies within the east abutment vault.
Span 1
- Between beams 3 and 4, near the west abutment: a 1'-2" length of conduit is broken open exposing
a cable and standing water within the conduit.
- Between beams 11 and 12, near the west abutment: there are two detached empty conduits.
Span 2
- Between beams 20 and 21, at pier 1: cracked utility casing 17" long (see photo 42).
- Between beams 20 and 21, at pier 2: a section of the utility conduit casing is not continuous (see
photo 43).
- Between beams 27 and 28, at pier 2: two conduits are disconnected at the splice collars and have
exposed cables.
Span 3
- Between beams 32 and 33, approximately 25 feet east of pier 2: the southern conduit casing is split
and loose; however, the conduit remains protected by an additional layer of casing.
- Between beams 34 and 35, near pier 2: the utility support bracket is missing from the connections
to the beams; however, support is not necessary at this location due to its proximity with the support
at the end diaphragm.
Span 4
- Between beams 30 and 31, at midspan: several conduits within the cluster are disconnected at the
splice collars and have exposed cables (see photo 44).
- Between beams 34 and 35, near the east quarter point: utility pipes are in hard contact with a steel
plate bolted to the diaphragm opening (see photo 45).
Span 5
- Between beams 29 and 30, at the east abutment: the junction box is open with exposed wires and
the conduit near the backwall is disconnected from the junction box exposing the wires.
- Between beams 31 and 32, at the east abutment: the conduit near the backwall is disconnected
from the junction box exposing the wires.
- Between beams 35 and 36, at the east abutment: the conduit near the backwall is disconnected
from the junction box exposing the wires.
- Between beams 36 and 37, at the east abutment: the conduit near the backwall is disconnected
from the junction box exposing the wires.

from the junction box exposing the wires.
Pier 2
- North sidewalk: the sliding plate for the joint is missing a connection screw at the north end of the
joint.
- South sidewalk: the sliding plate for the joint is bent upward up to 1/2" with debris beneath the
plate.
- Between beam 29 and the northeast wingwall: the netting is torn and ineffective.
- At beam 30 adjacent to the east abutment breastwall: several eye hooks are missing and the
netting is detached for a length of 12'-0".
- Between beams 30 and 31: several eye hooks are missing.
APPROACHES
West Approach
- Typical: numerous uneven patches, radial hairline cracking at the manholes and numerous up to
1/4" wide longitudinal and transverse cracks and potholes up to 4" wide x 5" deep x 50% perimeter
mostly at manholes (see photos 52 and 53).
- Full width of the roadway x 12" long adjacent to the west abutment deck joint: up to 1/4" wide
transverse cracks with localized areas of alligator cracking and break-up.
- (DEF=S/A) Eastbound right shoulder along the west abutment deck joint: a 2'-6" long x 3'-0" wide x
6-1/2" deep pothole which partially undermines the drainage structure cover (see photo 54).
- (DEF=S/A) Eastbound travel lanes along the west abutment deck joint: potholes up to 16" wide x
10" long x 2-1/2" deep, partially undermines the west armor angle of the deck joint and
depressed bituminous patches up to 2' long x 9' wide x 2-1/2" deep (see photo 55).

REMARKS
East Approach
- Typical: scattered longitudinal and transverse hairline cracks throughout and radial hairline cracking
at the manholes.
- Along the east end of the abutment vault: up to 1/2" wide map cracking across the full width of the
roadway with a 9" long by 18'-0" wide by 2" deep pothole across the westbound lanes (see photo
56).
- Along the east edge of the approach slab: up to a 3" length of map cracking across the full width of
the roadway with some localized break-up including an 8" long by 2'-0" wide by 2" deep pothole in
the right eastbound lane (see photo 57).
- Near the crosswalk, in the eastbound lanes: areas of up 1" high hardened concrete debris bonded
to the pavement.
West Approach
Right two westbound lanes: there are a few local depressions, typically 3' long x 3'-0" wide x 5" deep (see
photo 58).
East Approach
East end of the east approach slab: the roadway is heaved and depressed 4" along the cracking across the
full width of the eastbound lanes (see photo 59).
- West approach, south sidewalk: the brickwork surrounding the catch basin in the sidewalk has a
4'-0" wide x 2'-0" long area of 1 3/4" heaving (see photo 60).
- East approach, north sidewalk: two, 6'-0" long x 4'-8" wide concrete sidewalk slabs are settled up to
2 1/2" with respect to the top of the wingwall (see photo 61). The edges of adjacent slabs have
been ground flush to reduce the tripping hazard.
- East approach, south sidewalk, along the east end of the approach slab: there is a 3/4" vertical
differential across the full width of the sidewalk. A concrete ramp has been placed at this location to
mitigate the vertical differential and reduce the tripping hazard.
- East approach, south sidewalk: two, 6'-0" long x 4'-8" wide concrete sidewalk slabs are settled up to
2 1/2" with respect to the top of the wingwall. The edges of adjacent slabs have been ground flush
to reduce the tripping hazard.

Span 3
- Beam 29, from pier 3 to the beam construction joint: the cracks on the north face have been sealed.
- Beam 32, between the construction joints and the next intermediate diaphragm to the west: The
bottom edge of the beam has up to 10" long x 6" wide x 1" high edge spalls due to apparent
collision (see photo 69).
- Beam 34, between the western two intermediate diaphragms: the underside of the beam has an
exposed stirrup with heavy rust due to insufficient cover (see photo 70).
- Beam 35, near midspan: the north bottom edge of the beam has a 4" long x 3" wide x 1" high edge
spall due to apparent collision.
- Beam 38, near the beam construction joint: the south bottom edge of the beam has a 6" long x 3"
wide x 1" high edge spall/gouge due to apparent collision.
Span 4
- Beam 29, near midspan: transverse hairline cracks with efflorescence that extend across the
underside and up the vertical faces up to 3'-0" high.
- Beam 30:
o At the west beam construction joint: the underside of the beam has four exposed stirrups
with heavy rust due to insufficient cover.
o Near midspan: the underside has longitudinal hairline cracks.
- Beam 31, near midspan: the underside has longitudinal hairline cracks.
- Beam 32, near the west beam construction joint: the bottom edges have up to 4" high x 2" wide x
2'-0" long edge spalls due to apparent collision (see photo 71).
- Beam 33, at pier 4: the bearing area of the beam is spalled 10" high x 3'-1" wide x up to 5" deep
due to contact with the north shear block (see photo 72)
- Beam 34, at pier 4: the south face along the edge of the end diaphragm has a 8" to 2'-0" long x
14'-0" high delamination with up to 3/4" deep spalling (see photo 73).
- Beam 35, at midspan: the underside has transverse hairline cracks with efflorescence and an
exposed stirrup with heavy rust due to insufficient cover.
- Beam 36, near midspan: the underside has longitudinal hairline cracks.
- Beam 38, at midspan: the underside has an exposed stirrup and rebar chairs with heavy rust due to
insufficient cover.
- Beam 38, near midspan: the underside has longitudinal hairline cracks.

REMARKS
- Beam 32 bearing: the elastomeric bearing pad has rotated counterclockwise up to 2" movement at
south end bearing.
- Beam 33 bearing: the elastomeric bearing pad has rotated counterclockwise up to 3-1/2" movement
at south end bearing (see photo 75).
- Beam 34 bearing: the elastomeric bearing pad has rotated clockwise with the north half of the pad
overhanging the east edge of the pedestal by up to 1-1/2" at north end.
- Beam 35 bearing: the entire elastomeric bearing pad has shifted to the east and overhangs the
pedestal by 7-1/2" by full width (see photo 76) resulting in a 17% reduction in bearing area (as-built
= 2'-6" wide x 3'-5 1/2" long pad).
- Beam 36 bearing: the elastomeric bearing pad has rotated clockwise with the north half of the pad
overhanging the east edge of the pedestal by 1" (see photo 77).
- Beam 37 bearing: there is a 1/4" gap over an 8" width with a penetration of 6" between the beam
and the elastomeric bearing pad at the north end of the bearing.
The interior diaphragm between beams 31 and 32 at the west beam construction joint in span 4 has a 4'-0"
wide x up to 7" high x 3" deep spall along the bottom (see photo 71).
Additionally, there is lateral misalignment with contact between the beams and shear blocks at the east
abutment which has resulted in spalling of the south shear blocks for beams 29, 30 and 31 in span 5.

REMARKS
ITEM 60 - SUBSTRUCTURE
The manholes in the sidewalks are very heavy and very difficult to open. Unauthorized access is possible
through utility openings in the west wall into the west three chambers. These openings are covered with fabric
netting which is easily removed. The west three chambers adjacent to these openings have a history of being
flooded and had 1'-0" of standing water at the time of inspection.
At the east abutment, there is homeless debris just adjacent to the pedestrian walkway (see photo 79).
See Item 60.1.m for condition of the vault walls and floor.
The utility openings in the west abutment backwall are partially closed off with loosely placed or partially
mortared bricks. The brickwork in these areas is often crumbling; however, the backfill remains stable.
There is a 3'-0" wide by 1'-8" high by up to 8" deep spall with exposed reinforcing which undermines one
anchor for the west armor angle of the deck joint (see photo 47).
The west abutment is protected with dumped rip-rap. At the south end, some of the rip-rap is displaced and
falling through a deteriorated timber bulkhead.
East Abutment
- Beam 29: south shear block has a 5" long x 1'-6" wide x 9" high corner spall with exposed
reinforcing (see photo 50).
- Beam 30: south shear block has a 5" long x 1'-8" wide x 7" high spall along the top west edge.
- Beam 31: south shear block has two edge spalls; 5" long x 4" wide x 1'-5" high and 8" long x 1'-8"
wide x 9" high with exposed reinforcing.

REMARKS
The chamber floors have clogged drains, with areas of standing water (see photos 26 and 27) and some
localized delaminations and spalls.
West Face
- North end at the bottom corner: a 1'-0" wide by 6" high by 4" deep spall with a plastic bottle.
- Between beam 15 through 17: pile cap has horizontal cracks with hollow areas at the top corners
(see photo 82).
- Between beam 26 and 27: pile cap has top face hollow area 4'-4" long x full bay width x 5" high with
extending horizontal hairline cracks (see photo 83).
East face
- Below beam 30: a 5'-6" long x 2'-6" high hollow area with 10" long x 10" high x 1 1/2" deep spall
with exposed rebar (see photo 84).
- Below beam 34: a 4" long x 3'-4" wide x 4" high spall along the bottom edge.
- Below beam 38: a 4" wide x 1'-0" high x 1" deep spall with exposed reinforcing.
- At the south end of the bottom corner: an 8" wide x 3" high x 4" deep spall.

Item 61.3 - Debris
There is trash and driftwood accumulating around the slope protection at the west abutment including a large
piece of driftwood which is wedged between the dumped rip-rap slope and beam 13 in span 1.
- Pier 3, east face (span 4), below beam 32, seventh row from the top: there is a section of timber rub
rail missing.
- Pier 4, east face (span 5), below beam 29, fourth row from the top: there is a section of timber rub
rail missing.
