Greetings of peace!

I am writing this letter as a formal change order request of the project

entitled “Gohang Diversion Dam” located at Montabiong, Lagawe, Ifugao for the
construction of a grouted riprap which was approved by your good office.

As per request of the lot owners who are affected by the said projects, the
construction of the grouted riprap be changed to elevated flume in order not to
cause damage to their properties specially their rice fields.

In this regard, may I humbly ask your good office to change the project from
grouted riprap to elevated flume so as not to cause damage to the properties of the
lot owners who are affected by the above – mentioned project.

Hoping for your kind consideration on this matter and looking forward to a
progressive transaction with your office.
