Guidelines:

1. This is Performance Task No. 1, integrated with CLVE 10 and Social Science
10.

2. The students are grouped accordingly and will follow the same groupings
for CLVE and Social Science.

3. As a group, students will decide a set of interview questionnaires by


anticipating possible concerns, issues and scenarios in a specified
community. Remember that one of the essence of doing research is for
the community to benefit out of the research project. Note that there will
be no limit with regard to the number of questions to be prepared.
4. In order to formulate the questionnaire, students will select at least one
from the list of common concerns below. Students may also consider a
second option, in case the first one is not observable in the said target
community.

5. Contrary to the list, students must be specific in the questionnaire in order


to have a well-defined research problem. See table below for the space
in writing the questionnaire.
5.1. All members of the group must download this file and save a PDF
copy of the same questionnaire for PT1. Rename the document
as/with SECTION_GroupNumber_PT1 (Ex. 10A_Group1_ PT1).
5.2. The group leader is tasked to upload the questionnaire (in pdf) for
PT1 in the Assignments submission tab. All members of the group
must be notified upon PT1 submission.

6. Read and understand the rubric. Make sure all criteria are satisfied in
creating the questionnaire. Please see STREAM Rubric on the last page.
