Discussion:

SAFETY
1. Learning From Incidents (Awareness Alert)
* Master read and discussed about the collision between shell tanker and bulk
carrier in Singapore Straits.
What happened?
- Shell TC tanker was transiting Singapore Straits, when an overtaking
bulk carrier experience an abrupt power failure and was therefore unable
to control her steering movement which attributed into a collision with
our TC tanker. There was no injury nor pollution from both vessel. The
extent of damage was confined to the port side aft quarter of the tanker
vessel and the bow of the bulk carrier.
Why it happened?
- Abrupt loss of power by the bulk carrier
- Failure to comply with Collision regulations while underway within a
Traffic Separation Scheme

- Unexpected change in course by bulk carrier minutes preceeding into


collision
Lesson Learned
- Decision making in complex situation whereby assessment and action to
be optimal(speed and course) in the eyes of the Masters of both vessel
to reduce minimal impact.
- Taking early and substantial action to keep clear as a ‘Give Way Vessel”
was prudent to avoid close quarter situation as per Collision Regulations
while navigating (transiting a Traffic Separation Scheme).
- Crucial emergency preparedness during such incident to ensure
minimum impact are well verse by both vessel.

2. LET ColReg Review Matrix month of February 2019

As per COLREG Review Matrix, Master discussed the International


Regulation for Preventing Collision at sea 1972 for the month of February
2019 Stated at the matrix. The ff. COLREG Rules was discussed by Master

Rule 2: Responsibility

Rule 7: Risk of Collision
Rule 8: Action to Avoid Collision

3. Personal Protective Equipment

Master reminds all crew for wearing appropriate completed PPE base on their
working area, ensure that the required PPE are properly wear especially at all
terminal not only for compliance but for their safety.

4. Near-miss Report

A. Scenario No. 1
* Description of Near-miss event
- Vessel berthed at Shell Tabangao Jetty 2 for loading ADO-B0, when OOW
safety rounds carried out on deck before cargo operation pilot ladder on sea
side still rigged which is prone to ship’s safety and security.
* Root Cause
- Did not follow proper procedures / Lack of training
*Corrective / Preventive Actions Plan
- Gathered all deck hands for a brief meeting then discuss the proper ship
security procedures and the standard procedures to be made after
disembarkation of pilot.
- Explained to all deck hands the hazard of the said near miss and the proper
implementation of the ship’s safety and security procedures

* Description of Near-miss event


- Engineer welding at engine room workshop with oily rags near the welding
machine. Engine cadet noticed that some welding spark went to oily rags, so
engine cadet quickly off the power and ask the engineer to stop for a while for
he will be clean-up first the area.
* Root Cause
- Lack of housekeeping
* Corrective / Preventive Actions Plan
- Check first the area or surrounding before doing welding job.

MAINTENANCE

1. Deck and Engine Maintenance


The ff. topics discussed by Master and took attention of the department head
for all priority and maintained the vessel good performance

 Cargo Operation – All equipment pertaining on cargo operation should


working properly and good condition, all certificate are valid and
frequency of inspection are complied.

 Engine Maintenance – All machinery and equipment are maintained


according to the makers manual, Maintenance checklist stated at the
SMS procedure should be execute accordingly

 Mooring Equipment – All mooring ropes and mooring equipment must


inspected by chief officer frequently, non-compliance and observation
should be report to the Master to take corrective action

 Documents – Updating of all documents of the vessel to the Officer and


Engineer in charge including PMS Report, and record of testing and
inspection of all machinery and safety equipment.

 Deck and Engine Inventory – Monitoring of materials, stock, tools and


spare of both engine and deck department

2. Maintenance of Accommodation ACU by EE & R Marine – Industrial


Services
* Provided A/C Technicians transportation tools conducted checked, inspection
and leaking test the system, and found leaking on copper tee due to cracked,
conducted pump down the refrigerant and conducted brazing gas welding and
rectified the leaking conducted final leaking test the system run tested the
system, charged additional amount of refrigerant using ship spare of refrigerant
tested the ACU plant into normal working condition.

3. Pumproom Bilge P/S Side

* Conducted check of pump room bilge due to not working P/S as per C/Off.
Cnducted test of port side, cannot reset possible short contact of reed switch.
Check of reed switch as found contacted by water, conducted drying by cloth
and continuity test, as found normal reading. Test by magnet good reading at
0.0. Pump room bilge port side at control module possible at frozen contact can
rectify on next anchorage. Starboard side bilge alarm is functional &
operatonal.

4. ACU High Side Piping # 2


* Conducted check of ACU high side piping due to leaking – as found minimal
crack at 90 degrees elbow copper tube. Isolate all isolating valve for ACU #2
and release the remaining Freon and change on compressor and condenser
unit. Conducted start gas weld to repair the crack at the elbow 90 degrees
copper tube. Conducted leak test after by charging of nitrogen at compressor
#2 high side rectified no leak was found. Conducted pump out and vacuum the
system up to -30 H5 compressor side and condenser unit. Charged of R-22
freon up to low side 50 psi / high side at 240 psi. Conducted testing and
functional and found satisfactory.

FOOD, HEALTH AND HYGIENE

Safety reminders discussed by Master for safety of all crew

 Pest control ( cockroaches,flies and rats )
3. Health Monitoring( BP) / BMI – Body Mass Index
Master reminded the second officer to continue monitor the health of the
Crew. The weight and height chart uses the National Institute of health’s
body mass index tables to determine how much your healthy weight should
be for your height. Conducted to the Crew, taking their weight and Height to
determine if the Crew are Obese, Overweight or Normal using the table
weight and height guide chart. The BMI is a measure of your weight in
relation to your height. Crew with a BMI of less than 18.5 are underweight.

A BMI of between 18.5 and 25 is ideal. Somebody with a BMI between 25


and 30 is classed as overweight. A person with a BMI over 30 is Obese.

SHIPBOARD MANAGEMENT

1. Duties and Responsibilities


Master discussed duties and responsibilities of shipboard personnel and officer
stated at SMS Procedure, all crew are advised to properly maintain their
assigned task completely, maintaining all safety equipment and all machinery
operational and working in good condition.

2. Updating of SIRE Observation Rectify


Master mentioned all sire observation and non-conformity, all pending items
was discussed to review what is the status of those observation. All Person in
charge are remind by Master for the action needed to close out the problem on
time before the target date. The vessel continuous seeking for the observation
and non-conformity to comply and rectify immediately. Vessel requisition will be
raise related in SIRE Observation when needed.

SECURITY

1. Failed theft on ore carrier in Jinting Anchorage, China


Duty crew onboard the anchored vessel noticed an unlit barge alongside and
raised the alarm. On searching the vessel, oil stains were noticed near the
MGO tank sounding and nothing reported stolen.
