AFFIDAVIT

I, ANNABELLE T. TAPNIO, of legal age, Filipino, married, and a resident of Purok 7, Brgy.
Mining, Angeles City, after having been duly sworn to in accordance with law, hereby depose and
state that:

1. I am the true and registered owner of a certain parcel of land, covered by Transfer
Certificate of Title (TCT) No. 045-2022007876;

2. I was issued Transfer Certificate of Title that was entered last August 30, 2022, at Registry
of Deeds Angeles City;

3. Upon perusal of the TCT after being released by the RD Angeles, it was discovered that the
name in the TCT was erroneously written as “Annabelle F. Tapnio”, instead of the true and
correct name per the valid ID presented as “Annabelle T. Tapnio”.

4. The said discrepancy in the TCT was an error done inadvertently;

5. I execute this Affidavit of Discrepancy to attest to the truth of the foregoing facts, and for the
correction thereof in the TCT by the Register of Deeds.
