AFFIDAVIT OF CLOSURE/MINIMAL INCOME

I, MARRY JANE DALIPE AJERO, of legal age, married, Filipino, with residence and
postal address at Mateo, Kidapawan City, Cotabato, after being duly sworn accordance with law,
do hereby depose and say:

1. That I am the owner of L AND M BAKERY AND RESTAURANT. situated at

Mateo, Kidapawan City, Cotabato,;
2. That I am executing this Affidavit of Closure in my said capacity;
3. That I execute this affidavit of CLOSURE to formally close down the registered
business L AND M BAKERY for the reason that the above mentioned business has
stopped its operation since January 2021;
4. That the said Restaurant will continue to operate even it has a minimal income but the
L AND M BAKERY were formally decided to close down;
5. That I am executing this affidavit to attest to the whole truth and
veracity of all the foregoing facts in support of my application for the
foreclosure of aforesaid business;
6. AFFIANT FURTHER SAYETH NAUGHT.
