To grant KADSA Esports Organizing Team permission to use the video you have participated in and/or
your photograph and profile on the terms set out herein, please fill in your details, sign and return this
agreement.

In consideration of your participation in the video shoot/photoshoot and our undertakings below, you
confirm and agree as follows:

1. My image, portrait or picture, appearance, likeness & form may be taken, recorded, and used by
the University (or any third party the University designates) for advertisements, publications, and
general marketing materials, and printed publications.

2. I am aware that for Internet safety and data protection reasons. Only the school, name, grade
level, designation and section can be mentioned.

3. I agree that I will NOT receive any money or other reward for my participation in these photos or
videos.

4. KADSA Esports Organizing Team will NOT share my personal information with any third parties.

5. We aim to comply with the Data Privacy Act of 2012 and cooperate fully with the National Privacy
Commission (NPC). In doing so, DLSAU is committed to striking a balance between your child’s
personal privacy, which is important to us, and ensuring that our legitimate interests as an
educational institution and our ability to fully and effectively carry out our responsibilities as such
are met. It is the entire agreement between us and cannot be amended in any way unless we both
agree to this in writing and sign the amendments. This agreement will be effective from the
earlier of, the date of my signature hereto or the date of my video shoot/photoshoot above.
