B) Hematologic Test

Overview

A) As the ECG result with the conclusion "concentric left ventricular hypertrophy with
segmental wall motion abnormally and depressed overall systolic function consistent with
coronary artery disease and grade 1 left ventricular diastolic dysfunction" showed, the patient’s
heart disease manifested the occurrence of her Cerebrovascular Accident (CVA).

B) The patient's type 2 diabetes mellitus caused her creatinine to be high. Blood vessels in the
kidney, including nephrons, can be harmed by having too much blood glucose, which makes
them less effective in maintaining the filtering system and removing waste from the blood as a
result of creatinine accumulating in the blood's circulation. Conclusion: Also, a significant
contributing factor to the patient's CVA or stroke was a higher creatinine concentration.

Medical Management

● Insert Nasogastric Tube (NGT); 1,500 kcal in equal feeding

● Hemodynamic monitoring: Continuous hemodynamic monitoring should be implemented to

avoid an increase in blood pressure.

● Neurologic assessment to determine if the stroke is evolving and if other acute complications
are developing

Drug Study (Prescribed Medicines)

1. Ferrous Sulfate “FeSO4” (generic name)

Brand names: Feosol, Fer-In-Sol, Ferrousal etc.
Indication: Ferrous sulfate is an iron supplement used to treat or prevent low blood levels of
iron (e.g., for anemia or during pregnancy). Iron is an important mineral that the body needs
to produce red blood cells and keep the person in good health.
Contraindication: should not use this medicine if have had an allergic reaction to iron
supplements, or if have a condition called hemachromatosis (iron overload disease) or
hemosiderosis (iron in the lungs), or any type of anemia that is not caused by iron deficiency
Adverse Reaction: iron supplements can cause stomach cramps, nausea, and diarrhea in
some people
Route: Oral
Dosage: oral liquid drops. 75mg (15mg Fe)/mL ; >50 years. 8 mg/day ; Treatment expressed as
elemental iron. 100-200 mg PO divided q12hr
