Madam,

Greetings!

I am MYLA R. AGUADO, a SHS-Teacher II currently teaching Social Science at Tiwi Agro-

Industrial School, Bagumbayan Sitio Agro, Tiwi, Albay requesting to be transferred to Naga National
High School, Naga, Tiwi, Albay as JHS teacher for the following reasons to wit:

- I am teaching in my present station for six (6) years with utmost dedication as a teacher and I’ve
recently known that they need an Araling Panlipunan teacher.
- I am a bona fide resident of Naga, Tiwi, Albay and it would be easy for me to be assigned to a
nearby place
- I have a 105 year-old father living under my custody.
- I have a child who has special needs.
- I am no longer happy in my workplace to the point that my mental health is compromised.
-I have experienced emotional torture for almost 3 years and I can no longer fulfill the duties my
profession demands to do so due to the shameful and traumatic experiences I encountered during the
faculty meetings, PTA meetings, and even on social media
-I am planning to seek a medical expert for medical advice regarding my mental health concerns.

In light of these reasons, may I request to be transferred to Naga National High School. Rest
assured that I will continue to serve with utmost passion, dedication, and commitment to my profession as
proven when I became the SHS- Guidance Coordinator for five (5) years, SSG Adviser for four (4) years,
District Brigada Eskwela Coordinator for Secondary for three (3) years and currently a District School
Information Coordinator.

It is with sincere hope and prayer that this humble request will be given meritorious favor from
your very good office.
