Case Study: Entry into enclosed space

INTRODUCTION:
Entering in an enclosed spaces like ballast tank in a ship is a high risk, whether doing
some repair or cleaning purposes. For someone who wants to enter at the ballast, you must
be prepared and trained seafarer to do this task to prevent tragedy. Enclosed places like
this can risk our life. Even though shipping companies always issue warnings, there’s still a
numerous of incidents occur due to the failure of following correct entry procedures.

BACKGROUND:
Prioritizing to wear full protection (PPE) and preparing complete equipment is
necessary before entering a ballast tank which this two crew member desist to wear and
entered without any plan. They should already know how dangerous this place is or else
they didn’t undergo drills or training for this specific duty. Lack of preparedness is what
we’ve noticed here in this case. Even the air pipes is not working , still the 2nd Officer and AB
entered for inspection with only 2 open manholes. This improper setting is not applicable
for the crew member to work especially with inadequate ventilation and a limited openings
according to IMO. It’s a bit confusing how the 2nd Officer already feel uncomfortable in the
ballast but still didn’t go out or even call a help for the OS above. Poor decision making from
the OS. Instead of pulling an alarm to call a quick rescue, he even climb down at the ladder
and checked the 2nd Officer which is a very bad decision. Empty air bottles and not
calibrated oxygen content meter since 2005 is also a concern here. To summarize, the
Captain should be the one to be question here like in a voyage, he/she is the one who is
responsible for the safe and efficient operation of the ship. The company should also be
blame here in this incident for not conducting a ship inspection and recruiting an
inexperienced crew member.

CONCLUSION:
Following the safety precautions while entering an enclosed spaces will definitely
ensures the safeness of the seafarer. Planned and prepared crew member is a must when
entering in a high-risk setting like ballast tank to prevent this incident like this again. To
minimize the possibility of accidents in entry in enclosed spaces and identifying hazard, Risk
Assessment should be performed every time a man entry. Captain of the ship must face the
consequences for being lack of competitiveness and not doing his duty as a leader for
ensuring the safeness of his/her crew.
