Dear Fr. Richard,

After spending countless hours praying over my decision and consulting my spiritual director, I, Jan
Immanuel A. Tambaoan, would like to seek your permission to apply for re-admission to the Order of
Clerics Regular of St. Paul. Based on our initial conversation, I hereby promise to abide by whatever
recommendations you may have for me to continue my formation to the priesthood. This serves as my
formal manifestation of my desire to return to my seminary formation. Rest assured, that I will be more
open to the leadings of the Lord and be more forthcoming with regards to what I am experiencing so
that I can be guided positively.

The past years have never wavered my desire to serve the Lord as a priest. I tried studying for a regular
course, but the desire is still there. I hope it is not yet too late and that you will act positively on this
request.

I am looking forward to pursuing this application and receiving a positive response from you.
