Introduction: Leopold’s maneuver is a systematic abdominal palpation to determine the fetal presentation
and position. It is best done after 24th to 28th week of pregnancy, wherein fetal outline is appreciated and
uterine and abdominal muscles are stretched and thinned.
PROCEDURE 5 4 3 2 1
1. Wash hands prior to procedure.
2. Introduce self and identify the patient
3. Explain the procedure.
4. Instruct patient to empty bladder prior to procedure.
5. Place patient in a comfortable position (DORSAL RECUMBENT)
6. Drape patient and provide privacy.
7. Warm hands by rubbing one against the other before placing them on the abdomen.
8. MANUEVER I: (FUNDAL GRIP) to determine the fetal presentation. Which part of the fetus
is in the fundus.
 Facing the client, place palm at each side of the upper abdomen.
 Head: hard, round, movable
 Buttocks: soft, irregular, difficult to move.
9. MANUEVER II: (UMBILICAL GRIP) to determine the fetal position to detect location of fetal
back and extremities.
 facing the client, move hands downward over each side of the abdomen.
 While the other hand is stationary, the opposite should press gently the uterus.
Repeat palpation on the opposite side.
 Back: smooth, hard surface that offers resistance as you press inward.
 Extremities: irregular, number of angular modulations, lumps (hands, knees, elbows
of the fetus.)
 This maneuver is to determine FHR.
10. MANEUVER III: ( PAWLICK’S GRIP)
 Determine if fetus has engaged/ the fetal party us at the inlet and its mobility.
 Gently grasp the lower position of abdominal just above the symphysis pubis
between the thumb and index finger. And try to press the thumb and index finger
together. Determine any movement in whether the part is firm or soft.
 If the presenting part moves upward as an examiner’s hand can be pressed
together , the presenting part is not engaged ( not firmly settled into the pelvis)
 If part is prior, it is the head, then it is the breech.
 This maneuver is a confirmation of maneuver I.
11. MANUEVER IV: ( PELVIC GRIP)
 Facing the foot part:
 Place fingers on both sides of the uterus approximately 2 inches above the
inguinal ligaments.
 Pressing downward and inward in the directins of the birth canal. Allow
fingers to be carried downward
 This maneuver determines fetal attitude and degree of fetal extremities to the
pelvis.
12. Cover the patient’s exposed area during procedure.
13. Assist patient in changing after procedure.
14. Thank client for full cooperation.
15. Wash hands
