II. ENGAGEMENT
Hello ma'am Alexis Good Morning! I'm Ms. Cristine a social worker who works with individuals
and families; I am here to help those people who are experiencing same with your situation. I
have experience and encounter some cases like yours ma'am, so don't worry I will guide and
help you to get through this trying times you have. I will do my best to engage and talk to you
as often as needed for you to feel comfortable talking with me. I know how you feel right now, I
know how deep your pain you feel ma'am because everyone of us has difficult or trying times
so don't worry I will be right here by your side to help you get through this. Since today we have
receive a report about your case ma'am, we are here to help and hear your situations that
brought you here ma'am but if you’re still not yet ready we could be able to assess with you
until the time you’re ready to share with us.

Alexis reason that brought her to the agency:


1. Difficulty to solve and think about the solutions in her problem because she doesn’t
know where and what to focus first because of too much problems to face.
2. She becomes vulnerable because she can't control her emotions due to lack of people
who will feel the empathy with her.
3. She lost her control in solving the problem because of her emotional breakdown.
Therefore, she already needed to ask or seek help from people who knows better about
her situation and can help her about it.
III. ASSESSMENT
Since we are done with the engagement with the client and get to know about her. We will
jump to our next process of the case which is the assessment. In here we are going to know her
background and environment. Base on Alexis statements we could see she has this
environment:
1. They stay in a crowded and polluted area in the city
2. Their neighborhoods have different quarrels that sometimes affect them.
3. The person has been gossiping about Alexis situation due to her separation or she has
people gossipers in her environment. Also, according to some of her close friends and
relatives Alexis is taking up seriously every situation she has even simple mistakes
because this is how her environment makes her realize about life. Many people in her
surroundings is forcing her to do things that out of her limit already because they telling
her that she should work harder because of the situations of her child that need
medications for the treatment who is diagnosed with autism.
IV. TREATMENT PLAN
Social worker performs a full diagnostic evaluation:
 Social Worker give assess life stressors and health issues
 Social Worker selects appropriate treatment setting
 Social Worker helps to reduce overall frequency, intensity, and duration of the depression of
Alexis so that daily functioning is not impaired.
 Teach to Learn and implement coping skills that result in a reduction of depression and worry,
and improved daily functioning.
 Social Worker Verbalize an understanding of the cognitive, physiological, and behavioral
components of anxiety and its treatment.
 The Social Worker Describe the situations, thoughts, feelings, and actions associated with
depressions and worries, and the impact on functioning, and attempt to resolve his depression
problem.
V. IMPLEMENTATION
Social worker referred Alex's to a professional psychiatrist to help her in the situation that she is into
and to give her good treatment that will help her so that she can forget the complicated things she has
gone through in life, and so that she can return to her normal life without feeling of anxiety and
depression. And the worker will guide him in every step he takes. And Alexis will decide when she wants
to start the meditation or treatment plan that will be done with her so that she can recover and be
healed of whatever illness she is experiencing.
VI. EVALUATION
When the social worker and Alexis meet, Alexis told to the social worker that she does not have the
thought of suicide because she continues to take regularly the antidepressant pill as per the advice of
her psychiatrist and continues to visit and listen to the advice of her psychiatrist to avoid to the
inevitable such as over thinking, the gossips, and polluted areas because it might trigger her mental
illness. The social worker knows her situation of Alexis, the social worker becomes happy about her
progress of Alexis and Alexis feels the sense of hopefulness in her life.
VII. TERMINATION
When Alexis overcame her depression and was given all her needs for her recovery and she was
gradually put in good condition and a worker finished her job because she was given the treatment and
help she needed and for his child this is the time that the social worker needed to talk to her in depth
and explain to her that they need to terminate her and the social worker need to explain properly to the
client why they need to terminate her so that she does not be surprised and so as not to upset the client
because if we explain it properly the client will understand it and here is the time for a social worker to
do how they can terminate their client who is completely healed and given the help that is needed give
it to her.
