Why did the event happen in the first place?
Enumerate at least 3 factors that involve human factors and safety.
Come up with an idea on how to improve safety given the situation from the video.

Morales
The event was a tragic one, for the reason that it happened we can assume numerous
things. Firstly it was stated that one of the indicators was not working properly and gave
faulty readings thus giving false information to both pilots. Another reason it happened is
because of the other pilot not properly altering the captain. He noticed that the airplane was
banking at an steep angle but due to tradition of not being able to talk back or question the
one higher in command he was not able to alert him, The captain thus continued to bank to
get the indicator to respond but he managed to bank it to the point of no return and resulting
in a loss of control and eventually and airplane crash. One way we can improve safety is to
once the co-pilot notices the captain doing something wrong he should alert him especially if
it concerns human lives and an airplane crash. Piloting requires teamwork so the both pilots
should speak up if one notices the other is doing something wrong. The idea of hierarchy
should be scrapped when it comes to jobs that require great communication.

Mangosing
When it comes to aircraft mechanics, somehow the personnel played a part in the disaster
that happened with Korean Air aircraft. The attitude indicator malfunctioned for a reason.
Another reason why the disaster happened is because of the tradition of Koreans (since both
the captain of the aircraft is Korean as well as the First Officer), the personnel who are
above the other personnel are always in-charge, regardless of the first officer being right. For
the final reason, both pilots did not check the flight manual when they noticed the
discrepancy. Pilots should directly open and utilise the flight manual while they are
contacting the ATC.

Villon
The miscommunication between the captain and first officers caused the event. It affected
the pilot's decision to disregard the aircraft's warnings. One of the reasons behind this was
complacency. Even though we are comfortable with our jobs, we should not become
complacent about them. Always follow the protocol and precautions since they can save
your life and the lives of your passengers. Pilots must adhere to all alerts and take
precautions at all times, not only during emergencies. Because flying needs cooperation,
both pilots should speak out if one realises the other is doing something incorrectly.

Remoticado
Because flying needs collaboration, both pilots should speak out if one realises the other is
doing something incorrectly. When it comes to tasks that demand excellent communication,
the concept of rank should be abandoned. Whilst when it comes to aeroplane maintenance,
it is critical for any aviation company. It's critical for keeping a fleet in optimum shape, from
daily maintenance to overhauls - and keeping your maintenance staff safe and healthy leads
to more thorough work. A well-functioning and motivated maintenance crew guarantees that
aircraft are maintained properly. So in this field, aircfract maintenance experts' jobs save
lives, therefore they require the right tools, equipment, and a solid maintenance strategy.
Also, always maintaining excellent team communication is also a necessity, it means that
just because you haven't had an accident in your hangar in a while doesn't imply your crew
is immune to one tomorrow. By conducting frequent safety meetings and bringing up safety
problems anytime you transmit critical information to your employees, you will go a long way
toward being proactive rather than reactive. It's usually a good idea to ask your employees if
they have any suggestions for making their job safer. Your staff loves having their input
appreciated, and establishing a healthy work environment helps avoid complacency.
