• By filling this form, you authorize Sonata to carry out any kind of background check
regarding your Education, Past Employment, and Criminal Background and also carry out
Drug / Alcoholic tests if required by our client.
• Additionally, you would be required to provide Sonata any kind of information that we
require to carry out the above-mentioned checks.
• Please fill your Name, Date of Joining, Permanent Address and sign in the respective
columns.

2. Verification for the source of your recruitment

• We need to know the source through which your resume was forwarded to Sonata for a
suitable position.
• In the ‘’Source’’ column, please tick the appropriate Source.
• If referred by a Sonata Employee, please mention the complete name and employee id
of your referee.
• If you were referred by a Job Consultancy / Recruitment Consultant / Recruitment
Vendor to Sonata, Please mention the name of the agency.
• If you were hired through Campus Recruitment, please mention it as ‘’Campus’’.
• Please fill your Name, Date of Joining and Sign in the respective columns.

3. NDA - Employee Confidentiality & Rights in Invention Agreement

• Please fill the details of your Name, Permanent and Present Address in the respective
columns in the first page.
• Do go through the form in detail and then fill the details of your Name, Date of Joining in
the respective columns and provide your Signature.
• Under the ‘’Witness’’ section, please mention the Name and Contact Number of 2
witnesses.

4. PNDA - Personal Nondisclosure Agreement

• Please go through the form in detail and sign under ‘’Agreed & Accepted’’ header and
provide your Name, Date of Joining and Designation in the respective columns.
