Description of Incident: While off-loading a Mini Container from the Supply Vessel ‘Ed. Kyle’ owned by Phoenix Tide to Transocean’s Rig 702, 2 metal object was observed by the deck crew, to drop fram bottom of container onto the deck. ‘At time object dropped the container was at a height of 2 mts. above the deck. The size ofthe plate was approx. 48cm x | 24em x 2 mm thick, weight of plate 2.5 kg. | Note. The container was owned by Prime Leasing Port Harcourt and supplied in response to an urgent request by BGEPNL to SNEPCo, the container was noted as being fully certified. Synergi Case No: 9617.
