Incident Summary: Metal Object dropped from bottom of Container during lifting operations.
Description of Incident: While off-loading a Mini Container from the Supply Vessel ‘Ed. Kyle’ owned by Phoenix Tide
to Transocean’s Rig 702, 2 metal object was observed by the deck crew, to drop fram bottom of container onto the deck.

Note. The container was owned by Prime Leasing Port Harcourt and supplied in response to an urgent request by
BGEPNL to SNEPCo, the container was noted as being fully certified.

utilised by BGEPNL unless absolutely
necessary & then only after a full physical
inspection.
