HSSE - it’s our business Classification: incident reclassified 13/1/2009 as HIPO PMF Category 2P - Single Fatality Incident Summary: Metal Object dropped from bottom of Container during lifting operations. Description of Incident: While off-loading a Mini Container from the Supply Vessel ‘Ed. Kyle’ owned by Phoenix Tide to Transocean’s Rig 702, 2 metal object was observed by the deck crew, to drop fram bottom of container onto the deck. ‘At time object dropped the container was at a height of 2 mts. above the deck. The size ofthe plate was approx. 48cm x | 24em x 2 mm thick, weight of plate 2.5 kg. | Note. The container was owned by Prime Leasing Port Harcourt and supplied in response to an urgent request by BGEPNL to SNEPCo, the container was noted as being fully certified. Synergi Case No: 9617. Actions to prevent recurrence: / + Prime Leasing CCU's shall not be utilised by BGEPNL unless absolutely necessary & then only after a full physical inspection. + Feedback given to SNEPCo & Prime Leasing on findings of investigation. = In order to heighten awareness of the Ineed to report defective equipment, lfindings of incident advised to Base, |Vessel & Rig Crews.
