SUBJECT : PAINT RESIDUE MIXED WITH DRAINAGE WATER

Brief Description:

At around 1813H (September 30, 2022) I was informed through our Viber Group Chat that paint
residues were found mixed with the drainage water located at Main Power Station. I immediately inform our
Sub-Contractor, Sese Group, being the one who have an activity on that area. Upon speaking with Mr. JR
Sese and his foreman Mr. Jayson Tayag, I instructed them to follow me to the site to inspect the report.

As we arrived at the drainage area, in front of the power plant at around 1910H, I have confirmed
that it was actually a paint residue resulting from the washing of paint brush in the area.

Immediate Action:

1. Upon confirmation, immediately made contact with Mr. JR Sese and his foreman and informed
them of the incident.
2. Inform PM Ericson Marasigan and Ma’am Flor Ancajas of the incident.
3. Gave instructions to Foreman Jayson Tayag and JR Sese to bring drums and bucket to get or
drain the residue from the drainage. It is a good thing that the residue was controlled because of
good weather condition and the drainage was blocked by grasses scattered on the floor bed of the
drainage.
4. At around 2000H, fourteen (14) buckets were already full with the residue (see attached picture
A), but Mr. JR Sese was not able to produce drums and promised to provide it first thing in the
morning (October 01, 2022).
5. Control the water and residue by putting sandbag on each side.
6. October 01, 2022. The following day at exactly 1130H the drums arrived at the area.
7. At 1340H all residue mixed with water from the drainage were completely placed inside three (3)
drums and three (3) buckets. (See attached picture B)
8. Additional cleaning were done on the drainage such as removing of mud and cutting of grasses
on the base of the drainage. (See attached picture C)

Corrective Measure:

1. Discuss the matter/incident with the management and all key personnel.

2. Discuss the incident on toolbox.
3. Impose penalties on such behavior.
4. Inform the management on implementing Waste Management Control by hiring or designating a
competent person on this task.
