HAZID/ HAZOP WORKSHOP FOR

FEED KARAMBA FIELD PRODUCTION FACILITIES


BLOK WAIN – KALIMANTAN TIMUR

1. Background
Hazard Identification (HAZID) and Hazard Operability (HAZOP) has been conducted to identify potential
hazards, determine causes of the hazards, estimate qualitatively the consequence and likelihood of the
hazardous events and determine the appropriate risk reduction measures in order to reduce the risk to As Low
As Practicably Possible (ALARP).
The consultants (Puguh Mahendrodjati) have been engaged by PT. Pacific Process Engineering to conduct
the HAZID/ HAZOP workshops in order to identify the hazard of FEED – Karamba Field Production
Facilities, Blok Wain – Kalimantan Timur and determine if additional control measures and safeguards are
required. This report provides explanation of the HAZID/ HAZOP methodologies utilized in the workshops and
describe the recommendations which have been proposed during the workshops.

2. Objective
The objective of the project is to identify possible areas and to make recommendations as to how the particular
problems may be resolved. It is therefore critically important that clear actions and responsibilities are
established to ensure that the recommendations of the HAZID/ HAZOP analysis are reviewed and action taken
by the appropriate personnel.

3. Scope of Work
3.1 Scope of Work
The following scope of work shall be carried out to Hazard Identification (HAZID) and Hazard Operability
(HAZOP) Workshop and will cover facility of PT. Pandawa Prima Lestari:
1. Gathering and evaluate all documents/ drawings
2. Submit to COMPANY for timely approval HAZID and HAZOP nodes for the drawings related (i.e. EQL,
P&IDs, etc), worksheet to be used during HAZID/ HAZOP and action sheet template to record
HAZOP recommendation.
3. Chair and facilitate the HAZID/ HAZOP sessions;
4. Record the attendees at each HAZID/ HAZOP session on every day of the HAZID/ HAZOP workshop.
5. Provide a standard list of hazard groups and checklist/guidewords in accordance with COMPANY
Guidelines or ISO 17776 2000 (E) to stimulate discussion among the HAZID participants;
6. Employ the standard guidewords to each node and record the findings on individual HAZOP worksheets
describing the P&IDs, node identifier, and a short description of the node.
7. Identify risk ranking for each action identified as per COMPANY guidelines;
8. Prepare a HAZID/ HAZOP study report including a summary of action items and responsible party for
review and comment by COMPANY, and subsequent approval by COMPANY within agreed timescales

scope of work.
