Incident Summary

▌ Whilst checking the chemical drums in mini container at


open yard, LMP crew got hit by falling drum from upper part
row of mini container.

Medical Treatment Case
Accident Date & July 6th, 2014 at around 22:42 pm. At Open Yard
Location Lamongan Shore Base - East Java.

What Happen A 55 gallons plastic drum chemical fell from down upper
part of row inside mini-container and hit IP’s nose, resulted
fracture nasal on IP nose.

Why It Happen • Mini container doesn't have cargo net/ no bending and
poor arrangement, (refer to CLIENT Procedure PTS
602103 -- 4.2.10 Load carrier)
• Insufficient light during night
• IP not informed supervisor prior check material on the
container
• No drop object sign on the mini container

Corrective Action
Corrective Action Sending official recommendation letter to client to ask
them to implement their procedure PTS602103 section
4.2.10 “Load carrier” during cargo load, for any material
sending by container to HALLIBURTON (BAROID)
Sending official recommendation letter, asking client to
have checklist for inspection, related to mini container.

Inform shore base to improve lighting in relevant area of


operation.

Review BAROID work method adequacy related to work at


night shift
Communication between Supervisor and Employee needs
to be improved. Tasks to be carried out only after
discussing with LMP Supervisor
Sending official letter to client to put a “drop object” sign on
mini container
