 What is the phone number you’re calling from?
 What is your name?
 Tell me exactly what happened. (be concise)
For medical emergencies:
 How old is the patient? (approximate age is O.K.)
 Is the patient conscious?
 Is the patient breathing?
 Is the patient bleeding?
For a car accident or other emergencies that need the police:
 What is the vehicle description?
 What was the license plate number and state?
 What was the vehicle make?
 What was the vehicle model (sedan, SUV, truck, etc)?
 What color was the vehicle?
 Which direction was the vehicle was traveling
 Describe the suspect
 What was the suspect’s age?
 What was the subject wearing?
 Was the subject male or female?
 How tall was the suspect?
 Can you remember any other features?
Based on your answers, other questions will be asked. You may be asked to follow
some instructions before help arrives.
