Directions: Provide the following information for your class by responding to the following prompts (no more than 5
pages). Type your responses within the brackets following each prompt. Do not delete or alter the prompts.

1. With respect to the learning goal(s) from the learning segment, report on the content-specific
progress of your whole class on the formal assessment. Complete the table below to briefly
describe each learning goal and to provide the number or percentage of students who
exceeded, met, or have not yet met the learning goal(s).

2. Analyze the formal assessment rubric results for students in relation to the content-specific
learning goal(s) and ELD goal(s), if appropriate, and identify and describe patterns and/or
trends of student errors, skills, and understandings for the group or whole class and, in
addition, for the 3 students that represent a range of performance. Cite evidence from the
assessment responses submitted for the 3 students.
[ Based on the formal assessment rubric results, it is clear that all students were able to meet
the learning goals of this learning sequence. In the formal assessment, students were required
to identify the daddy number, correctly write the two fact family addition sentences, and correctly
write the fact family subtraction sentences. For the first learning goal, 19/20 students circled and

identified the correct daddy number among the fact family numbers listed. One did not circle, as
per directions, but he did use it as the daddy number in the number sentences so he was
awarded full credit, for clearly identifying and using the right number as the daddy number. For
the second learning goal, all 20 (100%) students wrote their subtraction sentences right with no
mistakes. For the third learning goal, 18/20 (90%) students wrote the subtraction numbers right
with no mistakes. 2 /20 (10%) students wrote 2 subtraction number sentences incorrectly.
Missing 2 still put these two students within the margin of meeting the learning goal. Student 1
correctly identified all daddy numbers, and wrote all addition and subtraction sentences
correctly. He scored a 12. Student two correctly identified all daddy numbers and correctly wrote
his addition and subtraction sentences as well. In his test there is evidence that he originally had
one problem incorrect, but he self corrected and got every question right. He also scored a 12.
Student 3 did not circle the daddy number as instructed, but it was evident from his calculations
and his equations that he put the daddy numbers in the correct spot, so I gave him full points for
that learning goal. He also correctly wrote all addition sentences and was awarded full points for
that learning goal. He made two mistakes on his subtraction sentences, so I scored him a 3. His
score is an 11. According to my results, I did not have a true, did not meet expectations. All
students met learning goals. ]

3. What did the results from the formal assessment tell you about what was most and least
effective about your instructional approach(es) in helping students achieve the learning
goal(s) and ELD goals, if appropriate?
[ The results from the formal assessment tell that that all the students in my class have a deep
understanding of how to find the daddy number within a fact family and how to write the four
number facts associated with the fact family. This is clear because every single student met or
exceeded all three learning goals. This shows me that my explicit instruction of how to find the
daddy number, and how to write the number sentences was extremely effective. Also, my
guided instruction, partner work, and practice opportunities through differentiated activities
proved to be effective in giving students the confidence to find the daddy number and related
facts independently. I believe the least effective part of my instructional approach was
dedicating a whole lesson to academic language. I believe that introducing the academic
language and beginning instruction with manipulatives would have enabled the students to be
more confident with the content sooner. ]

4. For those students whose achievement did not yet meet the content-specific learning goal(s)
and ELD goal(s), if appropriate, on the formal assessment, what do you need to do next
instructionally to meet their content learning and language development needs?
[ All of my students reached all three learning goals. However, the formal assessment allowed
me to see that 2 students were still somewhat unclear about subtraction sentences specifically.
This information allows me to plan a small group or individual instruction for both these students
to reteach the subtraction portion of the lesson. ]

[ The feedback I provide to the entire class will be positive encouragement about working
together and working independently to engage in activities to deepen our understanding of the
content and reach our learning goals. I will tell the class that the whole class accomplished this
together. My feedback will also include how the class succeeded in following directions, and
showing higher order thinking skills by discussing and explaining strategies. I will then connect
this concept to future learning and how understanding fact families will aid them in fact fluency
and number sense. ]

 Student 1?
[ Student one exceeded the learning goals and completed the worksheet with no errors. He was
able to correctly identify the daddy number, and write all four number facts correctly. While this
student achieved high marks and is confident in his skill, I will remind him to continue to check
his work for mistakes, and to show his math strategy when solving a problem. ]

 Student 2?
[ This student also exceeded the learning goals according to my rubric. He was able to identify
the daddy number, and write all four number facts correctly. The one thing I noticed about him
assessment was that he self corrected a problem. My feedback to him will include positive
acknowledgment of him double checking his work and finding mistakes. I will also encourage
him to show his work, in order to practice using math strategies. ]

 Student 3?
[ Student three met all the learning goals, though he made 2 mistakes on subtraction
sentences. My feedback will include following directions, as he did not circle the daddy number,
as instructed, but he clearly knew and used the daddy number in the correct position in
equation, so he was awarded full points for that learning goal. Also, I will provide instructional
feedback about the two mistakes he made on the subtraction sentences and how we can
strengthen his subtraction strategies.]

b. How will you assist the whole class and these individuals to use this rubric-specific
feedback to revise their work and/or take next steps to advance their learning?
[ I will assist the class by showing them how using strategies and showing work is helpful in
double checking answers. Since all students met the learning goals, I will encourage them by
explaining how achieving these learning goals will help future learning topics. ]
