Key Points
• Bidding System: Two Envelope
– First Envelope contains Administrative and Technical Data
– Second Envelope contains Commercial Data and Bid Bond
• Bid Proposal
– 1 set original Bid Proposal + CD/flash disk contain the softcopy
– Name and numbering of softcopy file shall refer to Form of Bid.
• Bid Validity: 3 months
• Bid Bond:
– Application: for Bid Price > IDR 1,500,000,000
– Value: minimum 1% of and in same currency as Bid Price
– Validity: 4 months from Closing Date
• No public bid opening meeting

Bumi (RUP) from Ditjen EBTKE (Energy Baru, Terbarukan dan Konservasi Energi) Ministry
of ESDM with the field of business classification in heavy equipment rental. In case Bidder
does not have the RUP, statement letter will apply RUP if appointed as the winner.
– Copy of valid Tax Payer Identification Number (NPWP) and SPPKP
– Copy of the latest Corporate Income Taz Return (SPT Badan Tahunan) and its official
reporting receipt from tax office
– Copy of VAT Return of the last 3 months and its official reporting receipt from tax office

– BPJS Ketenagakerjaan and BPJS Kesehatan Certificate

Scope of Work
A. General
• Contractor to provide heavy equipment for supporting Wireline Logging Preparation & Execution (major
activities) and additional well work activities at Company’s working area

• Company’s working area: Tapanuli Utara, Sumatera Utara


a. Silangkitang (SIL) Power Plant & Well Pads, Desa Silangkitang
b. Namora-I-Langit (NIL) Power Plant & Well Pads, Desa Simataniari

• Valid certification: Contractor must have a valid Surat Keterangan Terdaftar (SKT) atau Registrasi Usaha
Penunjang Panas Bumi from EBTKE

• Contract duration: 36months from Commencement Date

• Normal working hours : Monday – Saturday, from 08.00 – 17.00


Applied for heavy equipment & personnel (Operator & Rigger)

• Mob & Demob:


a. Contractor shall perform mob & demob its equipment to and from Company’s working area, including
proposed duration days of mob process to Company’s Working area. The price should be provided as
per Attachment 1 of Exhibit C
b. Inter-sites mobilization cost will be bear by Contractor with no additional charge to the Company

B. Equipment – Refers to B.3 Section 2 – Technical Data ITB
• Heavy Equipment specification:
a. One unit of 50Tons Rough Terrain Crane with telescopic hydraulic boom
Acceptable manufacturer: Tadano, Kato, Kobelco, Sanny
b. One unit of Truck Mounted Crane with lifting capacity of 8tons (FOCO for transporting 10” lubricator &
Geothermal Master Valves)
Acceptable manufacturer: Volvo, Scania, Hino, Mitsubishi, Isuzu
c. Year of manufacturing shouldn’t be more than 10 years old during Work Period (36months contract
duration)
d. Valid certificate throughout Work Period such as Surat Kelayakan Penggunaan Peralatan (SKPP) or
Surate Ijin Laik Operatsi (SILO) or Berita Acara Inspeksi .
e. Renewal certification will under Contractor’s responsibility

• Operation, maintenance and all consumable required for regular daily running of the heavy equipment are
to be provided by the Contractor

• Contractor to provide Technical Datasheet, Load Chart, List of standard lifting that will be used by
Contractor, maintenance plan program during Work Period

C. Personnel & Administrative Work
• One (1) crane operator, assisted by one (1) rigger for each heavy equipment unit.

• An operator (or replacement operator) must have valid license of Surat izin Operator (SIO) to operate the
crane

• A rigger (or replacement rigger) must have necessary permit (Sertifikasi Juru Ikat) as required by laws &
regulation of GoI

• Required license & permit should be maintained valid throughout Work Period at Contractor’s own cost

• The Contractor should provide resume / CV / experience for the proposed personnel

• If any personnel would be replaced during Work Period, Contractor should notify & obtain approval from
Company’s Contract Owner & Site Representative

• Contractor shall make their own arrangement for accommodation & transportation

• Contractor to provide PPE & perform annual MCU for the assigned personnel

• Contractor shall prepare daily timesheet being used by Company & submit for approval by Company
Representative, including Overtime Timesheet. The information at least covers duration, location, and
users.
