SYNTHESIS PAPER
Root cause analysis is the process of conducting an investigation and
analyzing problems. The investigators are needed to look outside the problem and
seek to comprehend the situation's or problem's root cause. This enables investigators
to minimize the recurrence of problems and develop solutions for their root causes,
which could be a problem with procedures, activities, or behavior. In addition, the
objective of RCA is to identify learning and areas that need improvements in order to
achieve successful results and prevent recurrence. Root cause analysis must be
undertaken systematically and supported by the requisite evidence for it to be
effective. During the early phase of the inquiry, a timeline or chain of events should
be established in order to discover and comprehend the relationships between causal
elements, the root cause, and the problem or issue at hand.

A cause and effect diagram, commonly known as a "fishbone" diagram, can be


used to discover potential causes of an issue and to categorize ideas into useful
groups. A fishbone diagram provides a visual method of analyzing data.  C ause and
effect It is a more systematic technique than other accessible tools for generating
potential reasons of an issue (e.g., the Five Whys tool). The problem or consequence
is indicated on the fish's head or mouth.  Potential contributing factors are indicated
under numerous cause categories on the smaller "bones." By encouraging the team to
examine the categories and investigate alternate causes, a fishbone diagram can aid in
uncovering potential explanations for an issue that might not have been considered
otherwise. Include team members who are intimately familiar with the procedures and
systems involved in the issue or occurrence under investigation.

Clearly, root cause analysis aids in preventing recurrence of incidents by


identifying and removing the underlying cause. In addition, it helps prevent the
recurrence of similar issues, expedites product development, decreases costs, and
improves product safety and dependability. Any company that wants to remain
profitable and competitive in the industry will consider RCA seriously.
