Ms. Muhammad Amira M.

Primary Care Provider, TLC Matters

1000 Therapeutic Way, Picket Fence, TN 30000

Dear Ms. Mohammed:

I would like to appreciate the dedication you have portrayed during the limited time you have

worked at TLC Matters but would like to bring to your attention some issues that would enable

you deliver better therapy services to the patients. I underscore that medication therapists are

concerned with the patient’s therapeutic outcomes improvement through medication therapy

review, medication record, treatment action plans, documentation, follow-up, or referral.

However, I have received some positive feedback from some of the patients you have handled

and the medical community around you on your perfect care services that have delivered positive

patient outcomes. To better and deliver enhanced care to your patients you need to understand

and espouse patient engagement skills and foster integration with your health care team. These

are pivotal skills that you need to possess and integrate in your care provision because it will

facilitate efficient patient-centric service delivery and therapeutic care coordination among the

medical community in your institution.

Although I have not had a firsthand engagement with you in practice, I would like to brief you

on the need to espouse empathetic patient engagement and integration skills throughout your

practice for better outcomes. Patient engagement will enable you to know and understand your

patients, their familiarities, and expectations out of the service to enable you structure and

schedule your therapeutic plan in a system encompassing their tacit therapeutic desires or

concerns. You must also explain and educate your patients about the role of other service

providers in the team who work in tandem to facilitate the success of the therapeutic plan.

Integration is a fundamental competence that medical therapy managers should possess. Medical

therapy management workflow is interdependent and consultative that requires continuous

exchange of information and care plans, medication prescription and administering, and

therapeutic monitoring and feedback. When communication and positive relationships prevail

the workflow proves iterative because a continuous exchange exists that supports efficient

reportage, feedback, and recommendations that consequently augments service quality,

guaranteeing optimal therapeutic outcomes for your patients.

I am optimistic that you will find time within your busy schedules to incorporate these add-ons to

your care provision.

Yours Sincerely
