AFFIDAVIT OF NON-OPERATION

I, RAFAEL ESTEBAN, of legal age, single, Filipino and with residence address at J.P. Rizal
St., Talisay, Batangas after having been duly sworn to in accordance with law, hereby depose and
state:

1. That I am the owner of RAFJANZ COMPUTER SERVICES with business address at W.

Trinidad St., Talisay, Batangas;
2. That the said business was registered under the Department of Trade Industry at Lipa City on
December 31, 2013 and valid until December 31, 2018;
3. That on December 2013 the said business is not operating;
4. That for this reason there was no income from the said business for the month;
5. That I am executing this affidavit to attest to the truth of the foregoing statements to inform
the proper authorities concerned, and for whatever legal purpose this may serve.
