INSTRUCTIONS: This activity is an individual activity.


1. Set a schedule for a family meeting that will discuss and create a plan for
your family in times of disaster.
2. Document the planning of your family.
3. Identify with your family, the different hazards that your household may
encounter.
4. Print out and display the important numbers.
5. Draw a map of your neighborhood and make sure that you will pin point an
evacuation area that everyone knows. Visit the site so that everyone will be
familiar with it or you can use the google maps to pin point your meeting
areas.
6. Notify the neighbors/friends that you identified that needs your help or will
help you in times of disaster.
7. Collate the emergency numbers. Place the directory where your landline for
easy access and viewing.
8. Gather materials for your GO BAG/EMERGENCY KIT and make sure
everyone knows where it is stored.
9. Make sure that every family member has a designated responsibility.
10. Your disaster plan should be updated regularly.
11. Lastly, ask *5 neighbors (it can also be your friends, relatives, loved
ones) or more to have their own Family Disaster Plan giving out a
softcopy of this template and explain the importance of having one.
Request them to do the same thing to spread awareness and build
cultures of preparedness and resilience in your neighborhood. (meet
them via zoom, google meet etc.)

THINGS TO SUBMIT:
You will submit a powerpoint presentation showing the whole process of your
family meeting and your family’s output. Likewise, include the 5 people that
you explained about the disaster plan. Do not forget to put labels / captions on
your photos.
