    To whom it may concern:

    Good day!

    I, Joymarie Salmoro Tomatao, a certified PhilHealth member, hereby waive Codeen Rie
    Tomatao Dalogdog, my daughter, as one of my beneficiaries. We have come to an agreement
    with his father, Kevin Mahinay Dalogdog, for him to transfer and include our daughter, as
    abovementioned, to be his PhilHealth beneficiary.

    I will no longer be covering any health-related medical bills in the name of my daughter. Thus,
    any benefits from PhilHealth acknowledging her as my beneficiary will be waived.
