I, REV. FR. ENRIQUE RELANO, of legal age and Filipino citizen after having
been duly sworn in accordance with law, do hereby depose and say that;

I am a team ministry member of Our Lady of Mt. Carmel Parish, Roxas City,
Capiz;
That based on our Parish records, Rachel Young was baptized on June 25, 1987
by Rev. Fr. Teodoro Tanalgo, appearing in our Parish Baptismal Registry series of 1985-
1988, No. 3/139/3;
That on May 3, 2021, our parish issued a copy of baptismal certificate of Rachel
Young indicating for marriage purposes.
I am executing this affidavit to attest to the truthfulness of the foregoing facts
and for whatever legal purpose it may serve.
