AFFIDAVIT OF LOSS

I, JOHN ALDWIN REANTASO, of legal age, married, Filipino, with residence at B7 L7 Crystal St.
Hilltown Residence, Brgy. Fortune, Marikina City, after being sworn to in accordance with the law hereby
depose and state that:

1. I am a former employee of Ayala Property Management Corporation, and the possessor of the
following document:

APMC Training Modules (Print-Outs), No CD Included during on boarding

2. On January 31, 2022, I discovered the loss of the said document under the following
circumstance:

Those documents were kept in our old house near my assigned property. After being
married, we transferred here in Marikina and did not notice that the documents were not with
me until now.

3. Despite diligent efforts, I could not find nor locate the same and I believe it is truly lost.
4. I am executing this affidavit for clearance purposes from Ayala Property Management
Corporation.
