 working hours  termination of service
our working hours are from 8.30am to 5.30pm and we during the probationary period or any extended period, your

 public holiday
we will observe all public holidays gazetted by the
respective Federal / State Government.
 sick leave
you will be eligible for 22 calendar days of sick leave,
no replacements will be given in case a public holiday upon certification by a registered medical practitioner or
falls on a non-working saturday. medical officer, which may be extended to 60 calendar

remember the employers’ benefit dollars? when employers embark on FlexBen, a certain amount of these benefit
dollars are converted into FlexCredits. With the FlexCredits that are allocated to you, you have the option of
determining which and how much of the available benefit options you need or want. there are six benefits options
currently available, namely: outpatient, dental & optical, hospitalisation, group term life, group personal accident ,
group critical illness and annual leave.

a ‘default’ for each of the benefit categories have been pre-determined for you depending on your marital / family
status. when making selections, you may choose more or less than your default. default selection will also be your
entitlement in the event you do not make any changes during the enrolment period.

you will need to have minimum coverage for outpatient, dental & optical, hospitalisation and group term life, to
ensure that your family and you will have some protection / coverage in the unfortunate event of illness, disability or
death.
group personal accident and group critical illness are optional benefits. you can select to have these coverage if
you have unspent credits. you may buy annual leave if you have balance unspent credits or sell annual leave to
obtain more credits.
after making all your selection, Unspent Credits will flow to a Flexible Spending Account (“FSA”). you may claim for
various items from the FSA.
you will continue to receive the Non-Flex Benefits that are outside the FlexBen Program.

how are my FlexCredits allocated?


FlexCredits are allocated based on the ‘price’ of your default benefits. the price of your default benefits are
determined, where applicable, on your monthly base salary, job grade, age and/or gender, eligible dependents, etc.
* therefore, it is important that you ensure your personal information is updated before enrolment. inform your HR for
updation and provide supporting documentation (eg: marriage certificate, birth certificate, etc.)

when is the enrolment period? when will the enrolment be effective?


for existing employees, enrolment period will be from Nov – mid Dec, for the next plan year beginning Jan – Dec (eg:
enrolment period from Nov – mid Dec 10, for plan year Jan – Dec 11). all new hires who join in Nov & Dec will be
covered with default coverage until the end of the calendar year, enrolment period will be provided for these
employees to select for the next plan year. enrolment form & insurance heath declaration form(s) if applicable have to
be submitted within 7 calendar days from enrolment date.

can I change my selections?


after the close of the enrolment period, you may change your selections only in case of a ‘life change event’ including
promotion, demotion, change in marital status, change in family status (eg: married with no children to married with
child(ren), change in no. of (eligible) children); if Critical Illness plan has been selected during the enrolment period
for the plan year, no change can be made for Critical Illness plan for the remaining coverage period within the same
plan year. annual leave buying and selling option is only available during yearly enrolment period in Nov-Dec for the
following plan year.

 exclusion list: medical / hospitalisation
the Company will not bear the cost of the following:

1. expenses for miscarriage, abortion and prenatal (pregnancy) or postnatal care and surgical, mechanical or
chemical contraceptive methods of birth control or treatment pertaining to infertility, erectile dysfunction and tests or
treatment related to impotence or sterilization are excluded from medical /hospitalisation;
2.expenses incurred in respect of child birth (including surgical delivery) are excluded from hospitalisation (however,
these can be claimed under Medical benefit subject to its terms and conditions);
3. expenses for treatment or surgical operation for congenital deformities and abnormalities, including hereditary
conditions, i.e. deformities present from birth such as hare-lip, club-foot, birthmark and any other congenital
anomalies;
4. expenses relating to plastic/cosmetic surgery, circumcision, refraction or surgical correction of nearsightedness
(Radial Keratotomy or Lasik), weight control/reduction/gain and the use or acquisition of external prosthetic
appliances or devices such as artificial limbs, hearing aids, implanted pacemakers and prescription thereof;
5. expenses relating to private nursing, rest cure or sanitaria care;
6. any expenses arising from treatment for illness, disease, condition or injuries sustained while committing a crime
or felony, while under the influence of alcohol, illegal drugs, narcotics, or mind altering substance or injuries which
are self inflicted while sane or insane;
7. any expenses relating to venereal disease and its sequelae, AIDS (Acquired Immune Deficiency Syndrome) or
ARC (AIDS Related Complex) and HIV related diseases, and any communicable diseases requiring quarantine by
law;
8. any expenses arising from provoked assault or breach of the peace or disorderly conduct, exposure to hazards or
participation in hazardous sports or activities except when endeavoring to save human life or property;
9. expenses incurred for sex changes.

10. expenses incurred for donation of any body organ by the employee or covered dependent and cost of acquisition
of the organ including all costs incurred by the donor during organ transplant and its complications;
11. investigation and treatment of sleep and snoring disorders, hormone replacement therapy and alternative
therapy such as treatment, medical service or supplies, including but not limited to chiropractic services,
acupuncture, acupressure, reflexology, bone setting, herbalist treatment, massage or aroma therapy or other
alternative treatment;
12. any treatment directed towards developmental delays and/or learning disabilities in employee’s children;
13. expenses of services of a non-medical nature, such as television, telephones, telex services, radios or similar
facilities, admission kit/pack and other ineligible non-medical items;
14. expenses arising from injury sustained from war or any act of war, declared or undeclared, criminal or terrorist
activities, active duty in any armed forces, direct participation in strikes, riots and civil commotion or insurrection;
unless caught in situation during business etc.
15. ionising radiation or contamination by radioactivity from any nuclear fuel or nuclear waste from process of
nuclear fission or from any nuclear weapons material.; unless caused by indirect nature of business
16. private flying other than as a fare-paying passenger in any commercial scheduled airlines licensed / registered
with Dept of Civil Aviation to carry passengers over established routes.
17.Psychotic, mental or nervous disorders, (including any neuroses and their physiological or psychosomatic
manifestations)

 exclusion list: GTLA and/or riders
the Company shall not be liable to pay in the event of illness or accident being directly or indirectly attributable to or
consequential upon:

1. active participation in any war (declared or not) or in any warlike operation, civil commotion, rebellion, insurrection
or military or usurped power unless the assured employee is serving a period of National Service;
2. attempted suicide, intentional self-inflicted injuries, use of drugs (except as prescribed under medical advice by
registered medical practitioners);
3. any intentional criminal violation of law;
4. expenses relating to private nursing, rest cure or sanitaria care;
5. racing of any kind (other than foot racing), professional sporting activities, and participation in speed, or
participation in extreme sporting activities for record-breaking, namely plane bungee jumping and sky skiing;
6. pregnancy or child birth unless disability continues for a period of three (3) months after the date of conclusion of
the pregnancy;

the insurance does not cover and no benefit shall be paid for disease caused, directly or indirectly, by any of the
following:

1. any pre-existing conditions;


2. any covered diseases / illnesses which are diagnosed within the first 90 days of enrolment / employment,
whichever is later;
3. war, invasion, act of foreign enemies, hostilities or warlike operations (whether war be declared or not), civil war,
mutiny, civil commotions assuming the proportions of or amounting to a popular rising, military rising insurrection,
rebellion, military or usurped power or any act of any person acting on behalf of or in connection with any
organisation actively directly towards the overthrow by force of any government or to the influencing of it by
terrorism or violence;
4. attempted suicide or self-inflicted injury whilst sane or insane;
5. any breach of the law by the insured or any assault provoked by him / her;
6. being under the influence of alcohol or drugs other than in accordance with the directions of a registered medical
practitioner;
7. aviation, gliding or any other form of aerial flight other than as a fare paying passenger of a recognised airline or
charter service;
8. participation in, or training for, any dangerous or hazardous sport or competition or riding or driving in any form of
race or competition;
9. involvement in any underwater activity;
10. injury caused by nuclear fission, nuclear fusion or radioactive contamination;
11. serving in any capacity for any navy, army or air force;
12. infection from any Human Immunodeficiency Virus (HIV), Acquired Immunodeficiency Syndrome (AIDS) or any
other AIDS-related condition, except for AIDS due to blood transfusion (as defined in the insurance policy).

invoice) upon submitting claims of the business travel items. Please
