SWORN STATEMENT

I, JOHN LESTER B BRUAN, of legal age, a resident of 1992 Purok 8, Baloc, Sto. Domingo,
Nueva Ecija, bonafide member PSFTP CL 2021-02 “MANDALASIG” and presently assigned at
Regional Service Training Unit 3, after having been duly sworn to in accordance with law, here by
dispose and state;

That on January 9, 2023 at 8 AM in the morning. While conducting our daily accounting at
RSTU 3 with the presence of RSTU 3 Staff, the undersigned report all the absents, Duty KP and Duty
Sentinel during that time;

That I found out that PT Coloma from station 2 is absent, So I immediately report it to RSTU 3
Staff and they told me to call PT Coloma to report to RSTU Personnel immediately right after to
continue our accounting of other station and That I found out that PT Ramos is also absent;

Moreover, I immediately called his Team Leader of Station 4 to find out where is PT Ramos;

That the Team Leader of station 4 call PT Ramos to find out where he is when he answered
the call, he told his that he was already at the station and like what I did to PT Coloma;

That I report it to RSTU 3 Staff that PT Ramos is also absent;

All of our accounting at RSTU 3 is accurate and true, I am not Lying, tolerating any of my
classmates because I know that is violation of PNPTS Training Guide.

I am executing this Sworn Statement to attest to the truthfulness of the foregoing facts and for
whatever legal purposes this may serve best.
