MEMORANDUM

1. Reference: Memorandum from CS, PNPTS re Notice of Explanation

received December 5, 2022 at about 10:41AM via messenger group chat.

2. This pertains to the above reference wherein the undersigned was made to
explain by the CS, PNPTS for failure to submit Weekly FTX Monitoring Report.

3. In this regard, please be informed that the documentation of RSTU3 in line

with the Field Training Exercise are being prepared by this unit religiously it just so
happen that the memorandum from the Director, PNPTS dated November 16, 2022
with subject: Regular Submission of Field Training Exercise Data was not receive in our
e-mail inbox but instead it was seen in our s-mail spam and because of these the said
memo was not routed and acted by the concern PNCO.

4. That the undersigned had already gave an instruction to immediately comply

on the same and make the necessary documents intended for the monitoring of our
Field Training Exercise following the format sent to us by the C, SFTTS. Rest assured
that said lapses on our part will not happen again and the timely submission of said
compliance will be supervised by the undersigned.
