a. PNP MC 2020-049 dated July 21, 2020, entitled, “Guidelines and
Procedures of the “Pensioner Ko, Sagot Ko” (PKSK) Project; and

b. PNP MC 2021-049 dated May 06, 2021, entitled, “Guidelines and

Procedures on the Creation and Maintenance of the PNP Pensioner Online
Accounting System.

2. Please be informed that at about (time and date), the undersigned conducted
(house visitation/Video Call thru FB Messenger) with PNP Pensioner (Name of
Pensioner) at the (Address/location).

3. In this regard, said pensioner was validated as (qualified) to receive Monthly

pension based on the qualification set by the PNP. Further, said pensioner updated his
contact information as a reference for the succeeding accounting cycle.To wit:

3. In this regard, said pensioner was validated as (disqualified) to receive Monthly

pension based on the qualifications set by the PNP. Further, the disqualification is due
to the reason that the pensioner (is Deceased, Reached the Age of Majority,is
Remarried/ engaged on cohabitation, changed his/her citizenship).
4. Attached herewith are the PIF and Photocopy of ID Card.
