Transfer Learning History Form Instructions

Purpose: This form can be used to submit a request to migrate learning history attached to a deactivated EID and merge it to the learning history of the curre
Active EID.

Instructions: Complete this form and send it electronically to the mHUB Service Center. If you need assistance submitting this form, go to mHUB > 'Contact

Important: Prior to submitting your request, please ensure the following:

- The new EID is active and has access to Learning in mHUB (search keyword 'Digital Learning' on MGS for additional information on Learning access).
- The previous EID had aceess to Learning systems and has prior learning history.

Instructions To Submit This Form Electronically:

1. Save this file to your computer and complete all required fields on the Form tab.
2. Submit the completed form to the mHUB Service Center following these steps (Design Hotels please call the Service Center directly to submit the form):
- Log in to mHUB
- Select the “Contact Us” link
- Select “Submit or View a Request” then Select “Submit a Request”
- Select the Topic “Learning” and the Sub Topic “Student/Manager” if you are a Learning Coordinator “Learning Coordinator”
- Type "Learning Transfer Learning History Request" in the Request text box
- Attach this completed form
- Click the Submit button

Requests for a single EID transfer of learning history will be fulfilled within 2-3 business days. Requests for multiple EID transfers of learning history will vary
according to the number of users. The mHUB Service Center will send an update when the request is complete.
