SITE VISIT AND INSPECTION CERTIFICATE

_____________________________ (Contractor/Company name), visited the site - MERALCO Theater, Ortigas Ave.,
Pasig City in connection with the Bid of Contract Package CP01 General Renovation Works for the Meralco
Modernization Project.

Having examined the bid documents, I certify that I conducted necessary due diligence acquainted with the nature,
geographical and exact location of the works; the general conditions required for the execution of work; the neighboring
building/s and other constraints that might affect the construction methods; physical conditions specific to the
construction sites; the climatic conditions; local conditions; means of communication and transport; the possiblity of
supplying water, electricity and other services; the availability of labor sufficient in number and quantity; all constraints
and obligations, all conditions and circumstances which might influence the execution or price of the works.

I further certify that I am satisfied with the desciption of the works and the Project Technical Group’s explanation and
that I understand wholly the works to be undertaken as specified and implied in the execution of the Contract.
