AFFIDAVIT

I, MEDARDO A. ABAD, of legal age, Filipino citizen and with

residence and postal address at 14B Sabater Compound Tanzang Luma II ,
City of Imus, Cavite after having been duly sworn to in accordance with law,
do hereby fully states that:

That I was a former employee of the City Government of Imus;

That last May 31, 2022 was my last day of service since I resigned;

I am filing for my terminal leave and therefore giving authorization to

deduct all financial obligations with my employer agency – City
Government of Imus, Cavite – that is audit disallowances, unpaid loans,
unliquidated cash advances;

I am executing this affidavit to attest to the truth of the foregoing

statement and that the same shall serve as my authorization for whatever
legal purpose it may serve.
