Creating a Case via Mobile Device

Employees with an open continuous leave of absence case will not be able to initiate a new
request until their previous case is closed. If there is no case open, follow the steps below to guide
the employee.

1. Ask the employee to log in to A to Z, guide them to click on More > Tools and
Resources.
2. To access the DLS portal, ask the employee to select Covid-19, leave of absence or
accommodations
As an alternative, guide the employee to Time> Disability and Leave Services (DLS)> View
details in the DLS Portal
3. Once the employee has accessed the Disability and Leave Self-Service Portal, the
main page will reflect the question Who would you like to open a request for?
Guide the employee to click on Select under the option Self.
Note: This option might not populate for all employees.
4. The employee will need to select the type of request that best fits their needs.

Important: Employees with an open continuous leave of absence case will not be able to initiate
a new request until their previous case is closed.

Note: Off-Work accommodations cannot be created from the Disability and Leave Self Service
Portal; however, it can be requested with the ERC-DLS Support team. Follow the Intake Process
(ERC-DLS Support): Accommodations for further guidance.
5. When requesting a Leave of Absence, the employee will need to select one life
event that best represents the reason of the request.

6. After the selection of the life event, the employee will need to fill the following
list of questions.
7. Recommend the employee to check the box to allow a detailed voice message. This
will ensure updates will come even if we are unable to reach them directly.

Note: The How would you like to be contacted for updates, might show defiantly as the
platform is undergoing some updates.
Creating a Case via Desktop Computer
Accommodation Request
If an employee requires an At-Work accommodation, follow the next steps to guide them
through the Disability and Leave Self-Service Portal.

1. Advise the employee to log in on the A to Z web page, guide them to click on Resources
through the drop-down section or at the bottom of the home page.

Advise the employee to select “Covid-19, leave of absence and accommodations” to be


redirected to the DLS portal.
As an alternative, guide the employee to Time> Disability and Leave Services (DLS)> View
details in the DLS Portal
2. The employee might get a log in request to access the DLS Portal as showing below.
3. Once the employee has log in, the main page will reflect the question Who would you
like to open a request for? Guide the employee to click on Select under the option
Self.
Note: This option might not populate for all employees.

4. The employee will need to click on Select under Accommodation at Work.


5. After the selection of the type of request, the employee will need to fill the following
list of questions.

Disability and Leave Self-Service Portal.

like to open a request for? Guide the employee to click on Select under the option
Self.

2. The employee will need to click on Select under Leave of Absence.

3. When requesting a Leave of Absence, the employee will need to select one life event
that best represents the reason of the request.
4. After the selection of the life event, the employee will need to fill the following list of
questions.
