1.0INTRODUCTION
1.1 BACKGROUND INFORMATION
Safety & Risk Engineering Solution (QSHE) Nigeria Limited has been contracted by
Waltersmith to develop operations safety case report for the Ibigwe flowstation.

As part of safe implementation of the scope of the project, a HAZID review would be carried
out to identify and assess all credible major accident hazards associated with the operations
of the facility, to ensure implementation of adequate risk controls and mitigations for the all
identified risks.

2.0 HAZID DEFINITION


HAZID (Hazard Identification) is a technique for identification of potential hazards and threats. It
is the process of identifying what can go wrong in a facility operations and deciding the best
control and mitigation measures to manage the hazard.
The major benefit of HAZID is that identification and assessment of the critical safety hazards
provides essential input to risk management decisions. This will lead to safer and more cost-
effective changes where applicable being adopted with a minimum cost of change penalty.

3.0 HAZID OBJECTIVEs


The objectives of Hazard identification (HAZID) study are;
 Provide identification of potential hazards, threats and operability issues associated
with the operations of Ibigwe flowstation activities.
 Assess the adequacy of safeguards, controls and mitigation measures in place to
effectively deal with these hazards and ensure risks are tolerable and As Low As
Reasonably Practicable (ALARP).
 Use Risk Assessment Matrix to assess the associated risksand identify major accident
hazards.
 Identify any action(s) required to eliminate or further reduce the risk associated with
these hazards to an acceptable level.

4.0 SCOPE
The HAZID will focus on hazards associated with operation of Ibigwe flowstation and
associated activities.

5.0 METHODOLOGY

HAZID Methodology
The HAZID session will commenced with a grounding presentation by Waltesmith Personnel
on the facility description, objectives of the study and the boundary conditions. This will be
followed by questions and clarifications. The hazard identification and assessment will
thereafter proceed through brainstorming exercise.

The HAZID worksheet will be projected during the session to enable team members
participate efficiently.

6.0 GROUND RULES


The under listed HAZID rules shall be applicable during the entire session;
• Put all mobile phones on vibrate / silent mode.
• Side discussions divide and distract the house. It shall be avoided.
• Prior to HAZID session, it is expected that participants would have read and
familiarised with HAZID Terms of Reference.
• The quality of the HAZID is as good and reliable as the level of contribution by
participants, therefore your full participation and commitment is required.
• Brain-storming is an essential aspect of the HAZID session but issues that cannot be
immediately resolved would be parked so as to save time
• HAZID session is not meant for design review; hence all drawings must have been
duly approved by the relevant authorities with evidence to show for it.
• Adoption of any recommendation is at the discretion of the Facilitator
• The HAZID Facilitator reserves the right to moderate the timing for tea breaks/lunch
and individual contributions depending on the energy levels during the HAZID
session.

7.0 ROLES AND RESPONSIBILITIES

All: Enforce ground rules, participate fully in the process but stay focused
and time sensitive.
To be present throughout the workshop unless previously agreed with
the facilitator (to avoid loss of input, back tracking, lack of ownership).
Facilitator: Manage the execution of the workshop. Lead and direct the process.
Scribe: Document the findings of the workshop. Display the record. Back up the
files regularly. Issue draft worksheets promptly.
System Leads: Be knowledgeable about a particular system.
Team Members: Actively participates in the identification and assessment process.

8.0 RECORDING AND REPORTING

During the study, all foreseeable and credible hazards that are identified by the team are
recorded on a HAZID Worksheet. Risk matrix below, shall be used to rank the potential risk
based on likelihood and severity to decide if the existing protection is adequate.

At the end of the study the outcome will be analysed and all major accident hazards identified
would be subjected to Bow Tie analysis to further reduce the risks to ALARP.

9.0 TEAM MEMBERS


The quality and output of a HAZID study is dependent on the experience of the persons that
make up the team. To achieve the desired output, the following core disciplines are required
to attend the HAZID session.

The HAZID session is scheduled to hold between 9.00hrs – 16.00 on Wednesday 11 th March –
Friday 13th March 2020.
