Foreword
Heritage Petroleum Company Limited (“Heritage”, “Company”) oil and gas land operations span
from the Southwestern Peninsula to South-eastern Coast of Trinidad, with non-operated assets
off Trinidad’s North and East Coast. It is a state-owned oil company involved in the business of
exploration, development, production, and marketing of crude oil locally and regionally.

Heritage has a corporate responsibility to ensure “no harm to people” and a statutory obligation
under Part II of the Occupational Safety and Health Act, Chapter 88:08 to ensure, so far as is
reasonably practicable, the safety, health, and welfare at work of all employees.

This Stop Work Procedure was prepared by the Health, Safety, Security, and Environment
(HSSE) Business Unit to provide guidelines for all workers on behalf of Heritage to ensure a
safe place of work for all. This is the first issuance of this document. It serves to ensure
conformance to our Control of Work Program requirements and compliance with relevant
regulatory requirements for under the OSH Act of Trinidad and Tobago. Heritage commits to
creating a culture where all stakeholders, including employees at all levels including contractors,
are empowered to stop work when there is reason to believe that someone’s health and safety
can be compromised. This is fundamental to the success of the Control of Work Program.

1 Introduction
The Stop Work Authority (SWA) is a fundamental element of a Control of Work (CoW) Program.
It’s applicable to all workers, both employees and contractors, at all levels within the
organization. SWA is best viewed as a safety policy or procedure that authorizes and empowers
workers to stop an action or condition, they consider to be unsafe. This encourages workers to
speak up without fear of retribution when they see a potential at-risk situation unfolding. It
empowers those closest to the work to make key decisions that directly affect their own health
and safety. This builds a partnership between management and employees and helps to foster
safety engagement and hence the development of the safety culture within the organization.

2 Purpose
The purpose of the document is to provide guidance on the Stop Work philosophy at Heritage. It
lays out clear guidance on each element of the Stop Work Authority inclusive of:

 When and how to stop work

 Reporting process

 Investigation

 Process for reinstating work.

 Communication with all stakeholders

This document, supported by the Heritage INSPIRE values, also establishes that there shall be
no management retribution, intimidation, or disciplinary action resulting from the act of stopping
work.

3 Scope and Exclusions


This SWA procedure shall be used in all areas under the control of Heritage Petroleum
Company Limited (Heritage). It applies to:

a. ALL work conducted on HPCL’s facilities, equipment, systems & premises

b. Every Heritage controlled site where there is a designated Heritage Technician,


Supervisor, Team Lead and/or Manager that have accountability over the site/area and
is expected to exercise direct control over the work/task details to be executed.

c. All employees, contractors, and their associated subcontractors regardless of their
position, level, or function.

d. Covers the means of safely controlling drilling & workover, construction, inspection,
maintenance, demolition, remediation, and similar work activities.

e. Applies to routine and non-routine activities.

f. Isolation Management, Breaking Containment, Gas Monitoring/Testing Management,


Confine Space Entry, Override Management, Working at Heights, Leak/Pressure
Testing, Excavation and Ground Disturbance and Lifting Management.

It is important to note where Heritage does not have operational control a comparable standard
shall be implemented. There are no exceptions unless otherwise stated.

5 Terms and Definitions


HIPO - An incident where the most serious probable outcome can be defined as a fatality

Incident - Any unplanned, undesired event that interferes with the orderly progress of work and
causes, or reasonably could have caused adverse health, safety, or environmental
consequences, damage to company property or reputation, business interruption, or
security/criminal acts

May - designates a Permissive Statement - an option that is neither mandatory nor specifically
recommended.

Near Miss - An undesired event that under slightly different circumstances could have resulted
in harm to people, damage to the environment or property or loss to process

Permit to Work - A formal, detailed document containing the location, time, and scope of work
to be performed as well as the equipment to be worked on, potential hazards associated with
the work, mitigations to be used, and the names of persons authorizing and performing the
work.

Shall - designates a Heritage Requirement and is used in Heritage documents where


conformance is mandatory.

Should - designates a specific Heritage Recommendation where conformance is not


mandatory.

Stop Work Intervention - An action taken by someone to stop work being performed or about
to be performed due to a perceived risk. The action can be direct by stopping the individual
performing the work or indirect by informing the person in charge to stop the work.

 Conform to this procedure


 Shall initiate stop work interventions when they believe it is warranted.
 Shall support stop work interventions of others
 Shall report all stop work interventions as per this procedure
 Shall clearly understand the hazards and risk related to the works being undertaken in
addition to the required control actions to prevent incidents from occurring.

Performing Authority

 Shall verify that employees understand that they have the obligation and authority to
“STOP work”.
 Shall resolve issues and concerns raised by Approving Authority, if necessary, before
work is resumed.
 Shall verify that all “STOP work” actions are properly reported, investigated and
corrective actions / follow up completed in accordance with this procedure.
 Shall ensure that lessons learned from “STOP work” interventions are shared when
determined necessary

Approving Authority/Issuing Authority

 Shall work with PA, if necessary, to resolve “STOP work” issues and concerns for
permitted work and the appropriate permit policies or escalate e the issue to the next
level of management if it cannot be resolved.
 Shall determine if the “STOP work” concern is a minor one-off issue that can be resolved
on the spot with no further action required or if the concern shall be considered a STOP
work intervention and be handled in accordance with this procedure.
 Shall resolve “STOP work” issues and concerns for non-permitted (routine, low risk)
work or escalate if the issue cannot be resolved

Line Manager

 Creating a culture withing work teams that allows employees the confidence to perform a
STOP work intervention when there is reason to believe someone on the work team can
be harmed.

 Provide adequate resources for the mitigation/elimination of known and unforeseen


hazards when they are highlighted

Operational HSSE Manager

 Shall develop and analyze data and trends from “s STOP work” reporting.
 Shall verify that lessons learned from “STOP work” interventions are shared when
determined necessary
 Monitoring and auditing the compliance to and effectiveness of the SWA Procedure

HSSE Leader

a) The HSSE Leader has the overall accountability for:

i. Ensuring this procedure is up to date, relevant and available to end users.

ii. The issuance, implementation, and administration of the outlined procedure to


ensure that STOP Work Authority is imbedded into the operation at Heritage.

b) Provide assurance that this procedure is being adhered to and the requirements outlined
are being rigorously and consistently met.

c) Ensure a self-verification program is in place to verify that the requirements outlined are
consistently met.

d) Approve and authorize any deviations from the set requirements contained herein.

e) Intervene and escalate as appropriate where STOP work authority is not being respected.

8 Heritage Requirements
8.1 Stopping Work

8.1.1 When to Stop Work

Work shall be stopped when any member of the workforce believes that there
may be an unsafe condition, act, error, omission that may result from a lack of
understanding, changes in conditions, job scope, method of execution, SIMOPS,
or any other scenario that is happening or about to happen.

8.1.2 How to Stop Work

8.1.2.1 The best way to STOP work is to call a “time out” and request
workers involved to stop work and listen to and discuss your
concerns.

8.1.2.2 Affected workers shall immediately be advised to stop what they


are doing by any member of the workforce that recognizes an unsafe
condition.

8.1.2.3 The PA or another member of the workforce shall then notify the
AA / IA of their concerns and “stop work” intervention

8.1.2.4 If the AA / IA determines that the “stop work” concern is a minor


one-off issue (e.g., not wearing hard hat, trip hazard created) and can
be resolved and corrected on the spot or if the concern is a
misunderstanding and not relevant, the workers can be advised to
resume work. This type of action shall not be considered a stop work
intervention and no further notification, reporting or investigation is
required. The issue should not be entered as a near miss, BBS entry
should be considered.

8.2 Stop Work Notification

8.2.1 If the “stop work” intervention is a major issue, then the affected workers
shall be advised that work cannot continue, and the work area shall be
restored to a safe condition. The IA, PA, and the AA shall agree on a course
of action to resolve the stop work issue. This can require reassessment and
changes to the PTW, the supplementary certificate(s) and the Level 1/2 Task
Based Risk Assessment (TBRA). If the activity is not a permitted job, then
changes to the Standard Operating Procedure (SOP) should be considered.
All changes shall then be communicated to the work team, via a 5x5x5-
toolbox meeting before allowing the work to restart.

8.2.2 If the “stop work” issue cannot be resolved by the IA, PA, and the AA,
work shall remain suspended, be properly secured to prevent an incident and
the issue shall be elevated to management for resolution.

8.3 Reinstating Work

Work shall restart when a reassessment of conditions and adequate control


measures have been implemented. Changes, if applicable, are then made to the
PTW, supplementary certificate(s), SOP and Level 1/2 TBRA as needed and
communicated to the work team via a 5x5x5-toolbox meeting.

8.4 Reporting

Stop Work” interventions shall be reported using Heritage Incident Reporting and
Investigation Procedure as a near miss. There shall be a clear indication within
the report that the near miss is a result of a stop work intervention.

8.5 Follow Up

If the “stop work” action meets the criteria for a High Potential (HiPo) near miss,
the Heritage Incident Reporting and Investigation Procedure shall be followed.

8.6 Feedback

In order to promote a positive culture around “stop work” actions, employees who
exercise their “stop work” authority should be thanked and kept informed of the
results of investigations and corrective actions.
