1. INTRODUCTION

This procedure details emergency contingency plan for this job site. Personnel involve in the
emergency procedure shall have an adequate training and practices to be carried out on
regular basis.
2. GOLDS AND SCOPE

This procedure has specific elements that detail emergency procedure to protect, minimize
impact of personal injuries or loss of assets. This document applies for activities of CIVIL
WORKS AND PIPE-RACK INSTALLATION.

5. AUTHORIZATION OF RESPONSIBILITIES
a. COMPANY

CPY Supervisor is a responsible to ensure emergency drill or exercise
performed regularly (monthly). He shall responsible to monitor the injuries
person is evacuated in safe manner.
b. CONTRACTOR
1) Contractor Respresentative/PJO
In case of emergency, The Contractor Representative and/or PJO have the same
responsibilities. They have the entire responsibility to initiate a Medevac after
consultation with the Medic on site. They also have a responsibility communicated
authority family injury person (IP)/victim, media and other relevant parties.

7. GENERAL EMERGENCY RESPONSE PLAN
This "General Emergency Plan" addresses emergencies involving construction activities:
a. Construction and support emergencies

c. General medical services and evacuations
Hospital medical support having a written agreement to provide fully medical assistance
to receive, care and cure of injury or illness employee are:
a. IMIP CLINIK
1) All emergency case(s) must be informed to CPY through SITE Emergency Phone
number and Clinic Office.
2) An effective communicator to answer the dedicated emergency telephone number
or radio channel in English or Bahasa Indonesia.
3) Emergency response team to do intervention to the injured person, secure the area
from any potential cause for secondary injuny, or other emergency situation
(equipment damage, fire or oil spill).
4) In injury or illness case, paramedic to respond the injured person before any attempt
to transport of injured person to site clinic. This is to prevent more severe to the
injured person.
5) CONTRACTOR provides Portable Fire Extinguisher unit throughout the construction
area where potential fire accident may exist.

8. ALERT & NOTIFICATION PROCESS
Medic will prepare the patient to Medevac from the site to Medical Facility; the following

9. RESPON TO HANDLE SCENARION POTENTIAL
a. Injuries and Illness
If immobilized, the injured or ill employees will be initially treated by a qualified
paramedic. General injury or illnesses cases will be treated at the Site Clinic; the
incidents shall be reported to Site Manager and Deputy HSE Manager through line
supervision. Off the job injuries and illnesses shall be treated either at the Site Clinic.
Serious injury or illness cases shall be stabilized before transportation direct to hospital.
After getting and advice from paramedic of the necessary to have a Medevac, PM shall
initiate Medevac. To minimize loss in the event of multiple serious injuries the
assistance of COMPANY medical personnel may be requested. For a life treat condition,
where a faster means of Medevac transportation is required such as transporting by a
special flight
b. Medical Evacuations
The paramedic shall evaluate the medical condition of injured or ill employee to
determine the possible need to Medevac. The requirement for evacuations sha!! be
notified to the Contractor Representative/PJO for approval. The locations to Medevac
personnel are CPY Clinic and IMIP Clinic. Medical details of the patient will be

detailed on "Urgent Medevac Information" form. Accompanying personnel will be one
Supervisor, HSE Supervisor. Regular reporting of the condition shall be forwarded to
the Site Manager, HSE Coordinator, Medical evacuations to Clinic is conducted
utilizing the standby Vehicle (Car) service. Travel time to CPY Clinic about five (5)
minutes and to IMIP Clinic about twenty (20) minutes. In unlikely event of multiple
serious injuries occurring, the number of intensive facilities at the Job Site is limited
and medical evacuation is considered essential.
c. Environmental Spills
Contractor committed to protect the environment. All significant spillages that are
potentially harmful to the environment must be reported immediately Maintenance
and Equipment. Supervisor is responsible to maintain adequate stock of containment
and disbursement equipment for the operations performed.
d. Fire
In the event of minor fire in the construction area portable dry chemical fire
extinguishers shall be mostly utilized. Fire extinguishers are located in near location
hot work area; Fire extinguishers are regularly inspected. When a fire is beyond the
control of local resources, the philosophy is to protect personnel by keeping well clear
of the fire.

11. FIRST RESPONDER, EMERGENCY CHECKLIST
a. Stop, to survey and evaluate the scene for potential existing or developing hazards
before a pproaching, e.g. pending structural failnure. Some hazards may be invisible,
e.g. lack of oxygen, toxic and explosive atmospheres or chemicals, and electrical
current. Do not become the next victim, proceed only when known safe to do so, this
may take time to confirm.
b. Call for Emergency help or delegate this task. See above for contact details and
requirements.
c. Care of the Injured. If you are qualified, unconscious patients need to be evaluated
for, airway, breathing, circulation (or heart pulse). All serious bleeding requires
pressure bandages and fractures to be immobilized. Avoid contact with blood. Do not
attempt to move or transport the injured. Serious injury or death can result. wait for
the qualified paramedic. The only exception being if more serious injury or death is
certain if the patient is not moved immediately. Do not allow an accident scene to be
altered. The only exception is to facilitate the recovery of injured or to make the area
